2015 Everett PTSA Council Awards Banquet
Your PTA unit’s Golden Acorn, Outstanding Educator, Outstanding Advocate, Student Advocate and Honorary
Lifetime Membership recipients for 2015 will be honored at our annual Awards Banquet on May 27, 2015.
The attached recipient invitation does not include a banquet cost, nor ask for an RSVP. It is each PTA’s responsibility to send the appropriate banquet fees and menu selection for each of your award recipients.
If any of your recipients are not able to attend the banquet, Everett PTSA Council still needs
the registration information for our records.
The Awards Banquet program also lists each new award recipient by name for each PTA, even if not attending the banquet. For accuracy, please indicate GA, OE, OA, SA or HLM award. Please list complete email and
home addresses for all recipients. Addresses will be used only to invite recipients to future banquets if we do
not have an email, and email information is appreciated so we may reduce the cost of future mailings.
Recipients are allowed to invite guests, either at their own or your local unit’s expense.
Payment and registration for a guest must accompany this reservation form.
Please include name and menu selection of extra guest.
Your registrations includes questions to ask each recipients so that others maybe get to know them. These
will be displayed during our school roll call, and will serve as your recipient’s introduction at the banquet.
