UPDATED PHS 398 APPLICATION FORMS, INSTRUCTIONS AND APPLICATION GUIDES
New application instructions and forms for paper-based PHS 398 applications and new application guide instructions for
electronic SF424 (R&R) applications are now available:

Some of the changes include:
• Supplemental Grant Application Instructions – is now a separate guide.
(Parts II and III have therefore been removed from the PHS398 Application Instructions and the general
Application Guides for Forms-B and Forms-C application packages.)
• Notice of Proprietary Information – Clarifies the policy regarding the final determination of whether an
application contains proprietary information and that a positive finding of proprietary information does not
automatically shield the information from release in response to a Freedom of Information Act (FOIA) request
should the application result in an award.
• Biographical Sketch – “Patent Citations” have been added as part of section C, Selected Peer Reviewed
Publications.
• Letters of Support – Clarification on the required content.
• Appendix – Photographs or color images of gels, micrographs, etc. are no longer accepted as Appendix material.
Such images must now be included in the Research Strategy.
• PHS 398 Applicants – All PHS 398 paper application submissions intended for due dates on or after September
25, 2013, must use the 8/2012 version of the PHS 398 instructions and forms. In addition, in the Research Plan,
Inclusion Enrollment Reports have been renamed and modified and are now “Planned Enrollment Report” and
“Cumulative Inclusion Enrollment Report”.
• SF424 (R&R) Applicants – Applications using electronic forms will transition to new forms identified with a
Competition ID of FORMS-C. In addition, The SF424 (R&R) guide contains a new Chapter - Supplemental
Instructions to the SF424 (R&R) for Preparing a Multi-Project Application.
The new application instructions and forms must be used for applications intended for due dates on or after
September 25, 2013. Any application submitted using incorrect application forms (including applications that have an
incorrect mix of old and new forms) may be delayed and may not be reviewed.
o
MODIFICATIONS TO NIH’s PLANNED AND CUMULATIVE INCLUSION ENROLLMENT FORMS
NIH has modified forms used to provide information regarding planned enrollment and actual cumulative enrollment of
individuals involved in clinical research studies on the basis of sex/gender, rate, and ethnicity.
Key changes include:
• The Planned Enrollment Report Form will now include the “More than one race” category.
• The layout of both the Planned Enrollment Report and the Cumulative Inclusion Enrollment Report Forms has
been modified to reduce confusion about racial and ethnic information being distinct concepts.
• The Planned Enrollment Report and Cumulative Inclusion Enrollment Report are now structured data forms.
• The modified reporting forms will be phased in starting with competing applications submitted electronically
following the timeline of NIH’s use of the updated electronic applications (FORMS-C). Non-competing
continuation applications will continue to use the current forms for inclusion data provided in the PHS 2590 or
RPPR until further notice.

Exemptions:
Unless otherwise required by department or agency heads, research activities in which the only involvement of human subjects will be in one or more of
the following categories are exempt from this policy:
Exemption (1) - Research conducted in established or commonly accepted educational settings, involving normal educational practices, such as (i)
research on regular and special education instructional strategies, or (ii) research on the effectiveness of or the comparison among instructional
techniques, curricula, or classroom management methods.
Exemption (2) - Research involving the use of educational tests (cognitive, diagnostic, aptitude, achievement), survey procedures, interview procedures or
observation of public behavior, unless:
(i) information obtained is recorded in such a manner that human subjects can be identified, directly or through identifiers linked to the subjects; and (ii)
any disclosure of the human subjects' responses outside the research could reasonably place the subjects at risk of criminal or civil liability or be damaging

NIH considers the sharing of unique research
resources developed through NIH-sponsored research
an important means to enhance the value and further
the advancement of the research. When resources
have been developed with NIH funds and the
associated research findings published or provided to
NIH, it is important that they be made readily
available for research purposes to qualified
individuals within the scientific community.(Data
Sharing Plan, Sharing Model Organisms, Genome
Wide Association Studies)
Users must prepare a document containing the
required information and upload it as a pdf file.
The Narrative Type requires Description Title in
the “Module Title” Field. (Do NOT use any
special characters or spaces in make you type
in COEUS)
A maximum of 10 PDF attachments is allowed in the
Appendix. If more than 10 appendix attachments are
needed, combine the remaining information into
attachment #10. Note that this is the total number of
appendix items, not the total number of publications.
A summary sheet listing all of the items included in
the appendix is encouraged but not required. When
including a summary sheet, it should be included in
the first appendix attachment.

In the budget justification, indicate general
categories such as glassware, chemicals, animal
costs, including an amount for each category.
Categories less than $1,000 are not required to
be itemized.
The proposal budget may request funds for the
costs of documenting, preparing, publishing, or
otherwise making available to others the
findings and products of the work conducted
under the award. In the budget justification
include supporting information.
In the budget justification, identify each
consultant, the services he/she will perform,
total number of days, travel costs, and the total
estimated costs.
The cost of computer services, including
computer-based retrieval of scientific, technical
and education information may be requested.
In the budget justification, include the
established computer service rates at the
proposing organization if applicable.

Modular budgets are applicable to certain research grant applications from domestic organizations requesting $250,000
or less per year for direct costs. International organizations and others that do not fall under this definition should use
the detailed budget forms.
Consortium/contractual F&A costs are not factored into the direct cost limit. They may be requested in addition to
the $250,000 limit. Modular budgets are simplified; therefore, detailed categorical information is not to be submitted
with the application. The modular budget is applicable only to R01, R03, R15, R21, and R34 applications. Note AHRQ
does not accept modular budgets.
For all modular budgets, request total direct costs (in modules of $25,000) reflecting appropriate support for the
project. There will be no future year escalations. A typical modular grant application will request the same number of
modules in each year. Provide an additional narrative budget justification for any variation in the number of modules
requested.
COEUS BUILDS THE MODULAR BUDGET FROM THE DETAILED BUDGET CREATED; THEREFORE THE DETAILED BUDGET IS ENTERED FIRST.
Modular Budget Expenses are mapped to the Coeus Modular Budget screen from the Detailed Budget Version marked

When completing the Project Role for the Investigator leading the portion of the project at the consortium site, the
project role of “PD/PI” should only be used if the entire application is being submitted under the Multiple PI policy. Also,
the role of Co-PD/PI is not currently used by NIH and other PHS agencies. Do not assign any individual this role. If
applicants wish to use roles of “Co-Investigator” or “Consortium PI”, select “Other” for the Project Role field and then
insert the appropriate role descriptor in the Other Project Role Category field.
Using Correct R&R Subaward Budget Form in your Coeus S2S Proposal
Given the various versions of R&R Subaward and R&R Budget forms available, we have developed the table below to
help you determine on which R&R Budget Form to enter the Subaward Budget Details in order for Coeus to translate the
Subaward Budget information correctly. Follow the usual steps to connect to Grants.gov in Coeus and in the Forms Tab
of the Grants.gov Submission Details Window, include the checkbox for the RR Subaward Budget and make note of that
specific attachment form name (e.g. RR Subaward Budget 30 V1.3). Then use the table below to determine which R&R
Budget Form version the Subawardee (or you) must use to enter the Subaward Budget Details:

TO EXTRACT THE R&R SUBAWARD BUDGET ATTACHMENT:
1. Locate and download the appropriate opportunity from Gants.gov website
2. Open the Adobe opportunity form set
3. Select the subaward form and move it to the “FOR SUBMISSION” box to open the form
4. Click on the “Click here to extract the R&R Subaward Budget Attachment” button to extract the form
5. Save the file locally using the fist ten letters of the consortium oranization’s name and use “.pdf” as the file
extension
6. Complete the required Budget entires (subwawrd budget requirements are identical to those of the proposal lead
budget) → Save
TO UPLOAD THE COMPLETED R&R SUBAWARD BUDGET ATTACHMENT TO THE PREMIUM PROPOSAL DEVELOPMENT BUDGET
1. Open the Proposal Details Screen
2. Edit
3. Select Budget → Click on the “Modify” button to bring you to the “Modify Budget for Proposal” Screen
4. Edit
5. Sub Award → “Sub Award Budget” window opens → Click on the “Add” button
6. Find the Subaward file, select it and click “Open”
7. Double click on the “Organization Name” field and enter the subcontractor’s name (no spaces or special characters)
8. Click on the “Translate” button (If the file has been generated successfully you should see the following: green
checkmark in the “XML” field, a system generated name in the “Attachments” field and a message stating “XML
Generated successfully” in the “Status” filed)
9. Click “OK” Button to close the window and return to the Budget
