2015 YEAR END AWARD
REGISTRATION FORM
Year End Championships will be awarded to the equine or horse/rider combination who earns the most points in their
division. Must attend three of the four shows to qualify. Please fill out the information below and give to the Secretary
before the end of the day with your payment. Champion, reserve, 3rd and 4th placings will be awarded to the winners at a
banquet luncheon held in the fall. Ties will be decided by total number of classes entered. Registration papers required for
breeds.
One horse/rider combination per form.
