All the information listed below is required for the scholarship committee to review your application.
1. Attach an official transcript of your college records. The college transcript must indicate your grade
average through the last semester completed prior to applying for this scholarship.
2. In 250 words or less submit a typewritten narrative indicating your present college plans and your
future career plans. This statement should include some of the following but is not limited to these
questions. What is your planned field of study? Why did you choose this field? What do you plan to
do career-wise? What are your goals or future expectations?
3. Any of the above supplemental information not accompanying the application will result in the
disqualification of the applicant from consideration.
