Program Overview
The Population Health Summit for Virginia focuses on current
approaches, plans, visions, and opportunities that can guide clinicians,
policy makers, healthcare leaders, researchers, and individuals interested
in implementing population health strategies to improve the care of the
citizens of Virginia.
Educational Objectives
Through participation in this summit, the participants have the
opportunity to:
n Identify and discuss initiatives currently underway in Virginia
that are related to population health.
n Create shared goals and/or initiatives across multiple healthcare
entities in Virginia for improving the health of Virginians.
n Provide opportunities for individuals from diverse healthcare
agencies in Virginia to network and collaborate toward a
common goal of improved health for its citizens.
Direct Providership Accreditation and Designation
Statement
The University of Virginia School of Medicine is accredited by the
ACCME to provide continuing medical education for physicians.
The University of Virginia School of Medicine designates this live activity
for a maximum of 7.75 AMA PRA Category 1 Credits.TM Physicians should
claim only the credit commensurate with the extent of their participation
in the activity.
Hours of Participation Statement
The University of Virginia School of Medicine awards 7.75 hours of
participation (equivalent to AMA PRA Category 1 CreditsTM ) to each nonphysician participant who successfully completes this educational activity.
The University of Virginia School of Medicine maintains a permanent
record of participation for six (6) years.

provider, endorses and strives to comply with the Accreditation Council
for Continuing Medical Education (ACCME) Standards of Commercial
