Some threatening calls are made to third parties such as television studios and newspaper offices. Anyone, however, may be on the receiving end of such a call. Bomb Threat Received by a Tenant - Should a bomb threat be received by a tenant over the phone, the following guidelines should be used: 1. Follow the Bomb Threat Record on the last page of this section. Try to obtain as much information as possible. Be prepared to relate this information to the police when they arrive. 2. Immediately call the Management Office at (303) 292-2300. Building Management will call the police. If possible, have a second individual call the Management Office while the bomb threat call is still in progress. Try to not use the telephone line that the bomb threat was received on so the police department can trace the call. Press “*57” immediately after the caller hangs up. 3. It is up to the Tenant to first decide if the threat is “credible" and to determine whether or not to evacuate their space. Upon receiving a bomb threat, the Occupant’s Floor Warden, or whoever is identified, should conduct a search in the suite to locate any suspicious article(s). You are more familiar with the contents of your own suite and, if spotted, are more likely to identify an object out of the ordinary. 4. If evacuation is required, follow the instructions on the following pages. 5. Be alert for any unfamiliar people and/or objects to point out to the Police or Building Staff upon their arrival. DO NOT touch or handle any suspected object. 6. Await the instruction of the Police Department and/or Building Management. The Denver Fire Department does not conduct a search of the property. The Denver Bomb Squad arrives at the property only upon notification that a device has been located. 7. Do not reenter the space until instructed. If you receive a call from the Management Office notifying you of a bomb threat, the Floor Warden should determine whether or not to begin immediate search and/or evacuation procedures. Bomb Threat Received by the Management Office In the event that a bomb threat is received by the Management Office, the following guidelines will be observed: 1. The Police Department will be notified immediately. 2. If a bomb threat is received against a specific floor, the Floor Warden in the affected area will be informed of the situation and Building Management will proceed immediately to the scene. The Floor Warden will give the order to evacuate the tenant space if necessary and will provide any further instructions. 3. Tenants should be alert for any unfamiliar people or objects to point out to the Police or Building Staff upon their arrival. DO NOT touch or handle any suspected objects. 4. The Floor Warden will make a complete search of the suspected areas to identify any suspicious item or items which do not belong in the space. 5. If a bomb threat is received against the Building, and not a specific floor, public access areas, i.e. lobby, restrooms, public hallways, etc., beginning with the most accessible, will be searched by Building Management. 6. An assessment will be made at that time. Bomb Threat Tenant Evacuation The Management Office will make a recommendation whether a tenant space should be evacuated. If your Floor Warden gives the order to evacuate, all of the following steps are to be followed: The Floor Warden will make sure all employees in your company are notified. Everyone should proceed quickly, but calmly, to the nearest stairway exit. DO NOT RUN! The Floor Warden or Alternate should walk the suite to assist employees and make sure everyone is aware of the evacuation order. Once the evacuation has begun, no one should attempt to reenter the evacuated area until it has been declared safe. A staging area, NOT IN THE BUILDING, should be designated off site. Floor Wardens should meet at the designated staging area and remaining occupants proceed to their staging area, where everyone should remain until further notice. No one should wander until directed by the Police or Building Management. The Floor Warden or Alternate should proceed to take a head count. If someone is missing, this information should be relayed to the Building Management. Emergency Procedures: Elevator Malfunction In the event that an elevator stops and you are in it, remember to remain calm. Pressing the emergency button within the cab will alert Building Security that the cab is malfunctioning. Please use the elevator phone to communicate with Security at the Lobby Desk. The Security Officer will continue two-way communication with passengers until help arrives. In the event of a power outage, elevators will continue to operate using our emergency power generator. Should an outage occur, elevator lights will remain on, but the car will temporarily stop. Each elevator will automatically return to the lobby where the doors will open and they will remain there out of service. Please remember to call the Management Office if you notice any abnormal elevator behavior so that we may contact the elevator maintenance company. IN THE EVENT OF A FIRE, ELEVATORS MUST NOT BE USED FOR EVACUATION. USE THE STAIRWELLS. Emergency Procedures: Emergency Action Plan To prepare for possible emergencies, Building Management works closely with Denver’s Fire and Police Departments to evaluate conditions, anticipate potential risks, and together have developed and maintain a comprehensive Emergency Action Plan. These procedures are not intended to alarm you, but rather to prepare you for any emergency that may occur while you are inside Colorado Plaza Towers. Denver’s Fire and Police Departments are equipped and trained to manage any incident. These building-specific procedures have been developed to present clear instructions on the actions required by you during the first minutes of an emergency, before safety personnel arrives. These actions are designed to reduce the potential for injury through calm and organized communication and reaction. While any emergency occurring in the Colorado Plaza Towers building could pose unique problems for building occupants and emergency personnel, thorough training in individual responsibilities and emergency procedures can minimize panic and should result in safe and efficient emergency response. To ensure safe evacuation of Colorado Plaza Towers in the event of an emergency, the procedures in this manual must be followed unless Fire or Police Department officials direct otherwise. All employees shall also follow the instructions of their assigned Floor Wardens. These procedures have been developed by Building Management, in conjunction with the Denver Fire Department, with your safety as the primary goal. All employees assigned to an office in the Building must read these procedures and become familiar with the actions required during specific emergencies. The importance of this Emergency Action Plan cannot be overemphasized. While it is understood that this plan cannot address every possible emergency situation, the guidelines set forth should be followed as closely as circumstances allow in order to reduce the chances of injury or loss of life. The primary purpose of this Emergency Action Plan is to establish a command structure, a sound decision-making process and effective communication lines and procedures for the occupants and guests of Colorado Plaza Towers. It is imperative that each employee become familiar with the emergency procedures handbook. If there are any questions, please call the Management Office at (303) 292-2300 before an emergency arises! Click here to download a complete copy of the Emergency Action Plan Emergency Procedures: Flooding In the event of a flood that may cause damage to Tenant property or affect the normal operation of the building, designated Tenant representatives will be contacted by Building Management personnel, including after business hours. The first priority is to ensure that no personal injury occurs as the result of a water leak or flood. The second priority is to limit damage to the property and to discover the cause and prevent or minimize additional flooding. Once the water has been contained, clean-up operations will commence. You are instructed to contact your insurance carrier immediately to request a claim adjuster to visit your office and assess for any damage to your property. Emergency Procedures: Medical Emergency Tenants Requiring Medical Attention Call the Paramedics - 911. Be prepared to provide: The address of the Building – Colorado Plaza Towers 621 or 633 17th Street, Denver, CO 80202 The floor and suite number. Call the Main Security Desk - (303) 962-9094. Notify the Floor Warden. The Floor Warden should take charge of the emergency until the ambulance arrives (unless another doctor or medical professional is on site). Attempt to comfort the injured party. Do not crowd around the person, give them space. Upon notification, the Security Team will alert the appropriate Building Staff which will: Open appropriate lobby or loading dock doors to expedite travel with a gurney. Bring the frieght elevator to the ground level to await the Paramedics of Denver Fire Department. Meet the emergency crew upon arrival and direct them to the appropriate area. Ambulance Service Once they have responded, the local ambulance service, 911 will automatically take the patient to the nearest medical facility. If another hospital is desired in non-emergency situations, consult the yellow pages in advance for alternative services. Have this number handy. Emergency Procedures: Power Failure Colorado Plaza Towers is designed to minimize the risk of a general power failure resulting from causes within the building. Typically, should a power failure occur, it will affect either an isolated area of the building or some larger portion of the downtown area. The stairwells and a sufficient number of fixtures in the offices are equipped with emergency lights powered by a generator, that will automatically go on and run for a limited time, in the event of a power failure. In the event of an electrical failure the following guidelines should be observed: 1. Notify the Management Office at (303) 292-2300. The power failure may be localized to your floor, so do not assume Building Management already knows of the failure. 2. If your telephone system is not operating due to the power outage, use a cellular phone to call the Management Office. 3. Open blinds/draperies to let outside light in. 4. Do not start an evacuation unless you are instructed to do so. If you are instructed to evacuate, lock all areas as you leave. 5. Do not congregate in the lobby areas or on the street. Move to your designated meeting area or go home, as directed by your company’s management. 6. If you are trapped in an elevator during a power failure, wait for assistance. Your elevator will cease operation, but will not fall. Do not try to force open the elevator doors or try to escape through the roof hatch. PLEASE TRY NOT TO PANIC. 7. The Management Office will attempt to advise you regarding the length and cause of the outage as soon as possible. Power Failure: Evacuation Procedures 1. The Floor Warden will make sure all employees within your company are notified. 2. Everyone should proceed calmly to the nearest exit stairwell. DO NOT RUN! 3. The stairwells and evacuation routes are equipped with emergency lighting. If possible, bring a flashlight along to help light the path. Do not burn candles. They are a fire hazard. 4. The Floor Warden or Alternate should walk the suite to assist employees and make sure everyone is aware of the evacuation. Emergency Procedures: Severe Weather While there are many types of severe weather categories, there are two (2) types of unusual weather conditions in Colorado which may occur and for which extraordinary precaution should be taken: Severe Thunderstorm Activity Tornado Watch/Warning Severe Thunderstorm Activity Local weather service will issue advisories predicting areas of probable severe thunderstorm activity and the estimated duration of such activity. The Building Staff actively monitors a weather band radio as well as national and local weather reports. No occupant will ever be required to remain in the building if they feel their safety is threatened. However, during severe thunderstorms, the building is often a much safer place than the streets. Never assume that you can beat a storm home. We recommend waiting the storm out in the building and proceeding home after the storm has subsided. Tornado Watch By definition, a tornado watch is an alert by the National Weather Service that a tornado can be expected in, or near, your area. Keep your radio or television tuned to a local station for information and advice from your local government and weather service. Also, keep watching the sky, especially to the south and southwest. If you see any revolving, funnel-shaped clouds, report them immediately by telephone using the 911 emergency number. Tornado Warning By definition, a tornado warning is an alert by the National Weather Service confirming a tornado sighting and location. The weather service will announce the approximate time of detection and direction of movement. Wind will be 75 mph or greater. Public warning will come over the radio, TV or the Municipal Defense sirens. Should a severe storm or tornado occur, the following guidelines should be observed: Move away from the exterior of the Building to a corridor or elevator lobby or any room without windows. As you move, close the doors of rooms which have windows. Also, be sure the door to your suite is closed tightly, but not locked. Go to the center corridor and protect yourself by putting your head closely to your lap or by kneeling to protect your head. Stairwells are safe. If crowded, move down to a lower level for shelter. DO NOT USE THE ELEVATORS. DO NOT go to the first floor lobby or outside the Building. Keep your radio or television set tuned to a local station for additional and updated information. KEEP CALM. If you are trapped in a perimeter office, seek protection under a desk. Once the weather has subsided, report any damage or storm related leaks to the Management Office, (303) 292-2300. Emergency Procedures: Toxic Hazards If there is a toxic spill or exposure, proceed immediately to an area where you are no longer exposed and call 911. Provide the operator with the building’s address, your floor and phone number, and also what type of spill has occurred. Then call the Management Office at (303) 292-2300. Take appropriate action to contain the hazard; close doors behind you, and always follow all safety procedures when working with toxic materials. Those persons with knowledge of the incident need to be available to the Denver Fire Department, Building Management and/or emergency personnel outside the building in order to answer questions. A description of what happened, where, when, and type of product(s) spilled will be extremely valuable. If possible, try to obtain the label from the product or container, or the MSDS sheet. The Denver Fire Department and Building Management will make a decision regarding how to proceed once all the facts have been received. Only when the environmental hazard is removed will the affected areas of the Building be allowed to be re-occupied. Restrict access to the area to prevent additional exposures and place exposed personnel together in a contained/controlled area. Emergency Procedures: Material Safety Data Sheets (MSDS) As required under OSHA's Hazard Communication Standard (29 CFR 1910.1200), please provide the Management Office with MSDS (Material Safety Data Sheets) for any and all hazardous chemicals you might use or bring on site as part of your routine tasks. Consumer products (copy machine toner, scouring cleanser, etc.) are not covered by the Standard. The MSDS should contain: A list of hazardous chemicals used or stored. An MSDS for each chemical. Introduction: History Colorado Plaza Towers 621 17th Street 621 17th Street, formerly known as the First Interstate Tower South, is a high-rise building in Denver, Colorado. The building was completed in 1957, and rises 28 floors and 365 feet in height. It stood as the largest and tallest building in Denver at the time of its 1957 completion, and held the distinction as the tallest building in Denver for eleven years until it was surpassed by the 420-foot Brooks Towers in 1968. 621 17th Street is considered to be one of the earliest modern skyscrapers in Denver. The original design of the building included an observation deck on the roof facing the mountains and a 4,430 square foot adjoining heliport. At the time of its completion, 621 17th Street was known as "First National Bank Building", its primary tenant. The building is located in the heart of Denver’s “Wall Street”, the financial district of the contemporary city. After First Interstate merged with Wells Fargo in 1996, the building's official name became its address. The 434-foot First Interstate Tower North, now known as 633 17th Street, was built adjacent to First Interstate Tower South in 1974. The two buildings together form Colorado Plaza Towers. 633 17th Street 633 17th Street, formerly known as the First Interstate Tower North, is a high-rise building in Denver, Colorado. The building was completed in 1974, and rises 32 floors and 434 feet in height. It also stood as the tallest building in Denver at the time of its 1974 completion, and held that distinction for four years until it was surpassed by the neighboring 709-foot 555 17th Street in 1978. At the time of its completion, 633 17th Street was known as "First Interstate Tower North" after First Interstate Bancorp, its primary tenant. After First Interstate merged with Wells Fargo in 1996, the building's address became its official name. Colorado Plaza Towers will be officially closed on the following holidays: New Year’s Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Day A front lobby desk Security Officer is on duty during each of these holidays. Should your office require cleaning, heating, ventilating, air conditioning or other special services on any of the above holidays, please contact the Management Office at least 48 hours in advance since the building staff and service providers also observe these holidays. Colorado Plaza Towers has security personnel on the premises 24 hours per day, 7 days per week. They are easily recognized as they are professionally dressed in uniform suits. Our Security Officers act as a deterrent to the would-be criminal as well as enforce building regulations, maintain order, and are on the alert for any unusual activities within and around the property. In order to receive this card please come to the Management Office (Suite 1650), with your completed and authorized access card form, which may also be obtained from your human resource department. This form must be signed by a supervisor prior to an access card being issued or access being altered. The card will be activated and ready for use within 24 business hours. This card will then give you access after hours to the building and onto your specified floor (where applicable). However, you will have limited access to the time and location that your supervisor has approved on your access card form. Access to the 633 17th Street building is available 24 hours per day, 7 days per week. Regular hours are from 6:00 a.m. until 6:00 p.m., Monday through Friday, and 8:00 a.m. to 12:00 p.m., on Saturdays (although an access system card is required for Saturday and Sunday entry). Access to the building at other times is restricted and is monitored by the building’s security systems and personnel. Entry to the building during times other than the regular hours previously mentioned requires tenants to use your valid access card to gain admittance to the building. Tenants must use the California Street entrance to 633 17th Street. The door to the right of the revolving doors has an after hours card reader. In order to gain access to the building swipe your access system card over the reader, this will unlock the door for you to open. Tenant Access System Cards To receive access system cards, the tenant should contact the Tenant Services Specialist in the Management Office at 303-292-2300. Access system card requests must be approved by an authorized contact for your company. Lost or stolen access system cards should be reported immediately to the Management Office. There will be a $25.00 charge to the tenant for cards that are lost, stolen or damaged. When an employee is terminated from the tenant’s employment, his/her access system card should be collected and returned to the Management Office along with a copy of the deactivation form. In addition, it is the responsibility of the tenant to collect the keys and cards of all terminated employees in order to maintain the security of the suite. Fitness Center The Colorado Plaza Towers Fitness Center also requires tenants to use their access system cards for entry. The Fitness Center, located in the 621 17th Street building, is intended for the exclusive use of Colorado Plaza Towers’ tenants, as well as the 700 17th Street building and their employees who have signed up and paid to be members of the Fitness Center. The Fitness Center hours are from 6:00 a.m. until 8:00 p.m., Monday through Friday, and 8:00 a.m. to 1:00 p.m., on Saturdays, closed Sundays. Non-Building Employees After normal business hours, tenant visitor(s) must check with the Security Officer on duty to gain access to the building. If the visitor(s) is accompanied by a 633 17th Street tenant, the tenant must log in his/her guest in and out in the building's logbook. If an unaccompanied individual requests admittance to the building, the security officer on duty will telephone the tenant’s office for approval. If guests/clients/visitors are permitted access to a tenant’s suite, the management office must receive notification in written form in advance, listing the individual’s name, company affiliation, purpose of visit, and approximate arrival and departure times. Tenant is responsible for leaving a key for such individual(s). Visitors will be required to show identification and log in to the Building’s logbook. If for some reason your card will not scan, the Security Officer will check the book to verify your specific permission levels. If access is allowed, the Security Officer will permit you into the building. If the book does not reflect proper access verification, the Security Officer will not allow you to enter. If access to the building is authorized and in the system and your card does not scan at the lobby desk, please bring your card to the Management Office any time between the hours of 8:00am to 5:00pm Monday through Friday with full detail as to the problem. Keep your card out of direct sunlight for long periods of time, and wary those washers and dryers. There is a $25.00 replacement fee if your card is lost, stolen, or damaged. Terminated Employees Any time an employee is terminated, a deactivation request must be sent to the Management Office along with their access card. If the termination is urgent and requires immediate deactivation, please call the Management Office, to give as much notice as possible. Policies and Procedures: Contractors RULES AND REGULATIONS FOR CONTRACTED SERVICE PERSONNEL The following rules and regulations have been adopted by the Building Management for the safety, benefit, and convenience of all tenants and other persons in the Building. The Contractor shall be responsible for notifying and enforcing their subcontractors, workmen and suppliers to observe all such rules and regulations. As a general rule, the Contractor shall make arrangements with the Building Management, as indicated below: Use of Loading Dock and Freight Elevators Material and equipment storage Time and place of deliveries After hours activities Building Tie-ins Special material handling and hoisting facilities Hours and coordination of work Power, heating and restroom facilities Clean-up Discrepancies in plans 1. Building hours: 6:00am to 6:00pm Monday through Friday and 8:00am to 12:00pm Saturday, excluding holidays. (subject to change by Building Management.) All Contractors, subcontractors, workmen and suppliers are required to use the loading dock entrance and freight elevator to gain access to and from floors under construction. It will be the responsibility of the Contractor to submit a request by 3:00pm to the Building Management Office for work after 6:00pm or before 6:00am the following day. Also, weekend activities must be coordinated by 2:00pm Friday. 2. Loading Dock: Access to the loading dock is available 24 hours with 24 hours advance notice. The delivery of merchandise, supplies, fixtures and other materials or goods of whatsoever nature to and from the premises and all loading, unloading and handling shall be done only at such times as designated and pre-arranged by Building Management. 3. Freight Elevator: The freight elevator is available for use by the Contractor from 7:00am to 6:00pm Monday through Friday (except on Wednesdays from 10:30am to 12:30pm.). All construction material deliveries and furniture/office moves must be scheduled after hours Monday through Friday or any time Saturday and Sunday with prior arrangements. All contractors and delivery personnel will be required to sign in and out if working before and after normal business hours at the dock office. Twenty-four hours advance notice is required on a first-come first serve basis and must be pre-arranged through the Building Management office. 4. At no time will Contractors, workmen and suppliers be allowed to transport equipment or supplies via the passenger elevators. Note: The property does not rent or loan tools or equipment for Contractor or Tenant use. 5. Construction crews will provide their own parking for all personnel. Loading dock rules prohibit parking in the loading dock. Unauthorized vehicles will be ticketed and towed at owner’s expense. 6. Emergency lighting and life safety systems shall not be disconnected under any circumstances without prior written notice from Contractor and with approval from Building Management and scheduled through the Building Managements office 24 hours in advance. Work shall be done expeditiously and emergency facilities shall be restored immediately upon completion. 7. Any work involving welding or the sprinkler system must be coordinated through the Building Management office at least 24 hours before work will be performed. 8. Contractor is responsible for turning off lights and breakers each night. 9. The Building Management shall be notified 24 hours in advance before Contractor cuts into any duct, water line, any existing power line or temporarily disconnects any meter, or before moving any air handling equipment, thermostat, etc. Additionally, 24 hour notice will be given to Building Management prior to any varnishing or use of toxic materials so that ventilation may be provided. (Use of toxic materials may require after hours work.) 10. Contractor is required to notify Building Management 24 hours in advance before Contractor does any core drilling, and all slabs must be x-rayed prior to drilling. All such work must be scheduled through the Building Management’s office, and such work shall be done after normal business hours. 11. A key for the janitor’s closet will be assigned to Contractor by the Building Management on a day to day basis. Upon completion of construction, Contractor agrees to thoroughly clean the janitor’s closet, including the waste lines, and to repair any damages. A representative of the Building Management will verify that this work was done. Any clean-up work or repair of damages not performed by Contractor will be undertaken by the Building Management at Contractor’s expense. 12. Contractor doing work on an occupied floor will be required to cover all finished floors and walls with minimum 6 mil. Clear visqueen until all major deliveries have been received and all drywall work completed. 13. Any access required into an adjacent Tenant space will have to be coordinated by the Contractor through the Building Management’s office. 14. All construction crews shall be confined to the tenant space at all times with the exception of the retail area during business hours. 15. Prior to commencing with construction, Contractor shall provide the Building Management with a list of the names of the subcontractors and their related trade along with emergency phone numbers of the project manager. 16. All access to and from the floor(s) for Contractor, subcontractors, workmen and suppliers where work is being done is restricted to the use of the freight elevator. At no time will passenger elevators be used. 17. Absolutely no smoking will be allowed inside the building or within the construction area. Smoking is allowed on the outside plaza in the designated smoking area only. No radios will be allowed on the construction site. It should be noted that the Contractor is responsible for their subcontractors, workmen and suppliers. Any action detrimental to the building and/or its tenants shall become the sole responsibility of the Contractor. Policies and Procedures: Floor Loads Building requirements prohibit placing loads upon floors that exceed 50 pounds live load per square foot in office areas. Should a Tenant/vendor find it necessary to utilize equipment that exceeds this rating, the tenant/vendor must receive prior written approval from the Management Office of the Building. Adequate documentation from a licensed structural engineer is required verifying that such an installation at a specific location is safe. The architect and the General Manager of the Building will review the Tenant’s request and accompanying documentation. When the Management Office receives confirmation that the installation is safe, they will send the tenant written approval. Policies and Procedures: Insurance Requirements Insurance Requirements for 621 17th Street I. The Service Contractor shall provide the following minimum insurance coverage: A. Commercial General Liability Combined Single Limit - $1,000,000 per occurrence Combines Single Limit: $2,000,000 Aggregate. Such insurance shall be broad form and include, but not be limited to, contractual liability, independent contractor's liability, products and completed operations liability, and personal injury liability. . B. Worker's Compensation - Statutory Limits C. Employer's Liability With minimum liability limits of $500,000 each accident, $500,000 disease, $5,000,000 bodily injury each employee D. Commercial Automobile Liability Combined Single Limit - $1,000,000 per accident. Such insurance shall cover injury (or death) and property damage arising out of the ownership, maintenance or use of any private passenger or commercial vehicles and of any other equipment required to be licensed for road use. E. Excess Liability in the form of an Umbrella Policy should equal $1,000,000 each occurrence and $1,000,000 aggregate. F. Property Insurance All-risk, replacement cost property insurance to protect against loss of owned or rented equipment and tools brought onto and/or used on any property by the Service Contractor. II. Policies described in Sections I.A. and I.D. above shall include the following as additional insured, including their officers, directors and employees. A GL-2010 Endorsement shall be utilized for the policy(ies) described in Section I.A. above. Management and Leasing Team will assist you in every way possible to ensure a smooth and comprehensive relocation. Any arrangements for various trades such as telephone or copier installation, etc. should be set up in advance through the Management Office. Tenant Responsibilities Prior to Moving Out - Tenants are requested to follow the procedures listed below before moving out: As soon as the Tenant is aware of a potential move, they must contact the Management Office to schedule the freight elevator. Contact the telephone company to discontinue service at this Building. Turn in office keys and access building and/or parking cards to the Management Office. Once all tenant items have been removed from the space, a walk-through must be scheduled with the Assistant General Manager and Director of Engineering. Follow the “Moving Policies” described on the next pages. Moving Polices In order for building personnel to accommodate your needs and to protect the building property, the following policies should be followed. Contact the Management Office if there are questions: Schedule Tenant move with the Tenant Services Specialist at (303) 292-2300 as far in advance as possible (minimum of two weeks). The move will be scheduled based on the availability of elevators and building service personnel, on a first come, first serve basis. Provide the Management Office with a letter listing the following information: Date of move. Time periods the freight elevator will be needed. Name of the moving company and the moving supervisor for the Tenant. Certificate of Insurance for the moving company that is current and in compliance. Forwarding address and telephone number of Tenant. Special Requirements The Management Office should be advised in writing of any special requirements in connection with the move. For example, if draperies, supplies, equipment, etc., are expected prior to the move, arrangements must be made for use of the freight elevator and for access to the tenant suite. General Procedures All items moved in or moved out must be taken to the loading dock and taken away immediately. Items are not to be stored on the loading dock. No items are to be moved across or through the main lobby floor. Movers must sign in with security at the 633 lobby desk prior to loading or unloading tenant materials or furniture. The mover will be required to provide identification and state the name of the Tenant being moved. Only the freight elevator is to be used. No moves are allowed Monday through Friday between 7:00am and 3:00pm unless previously authorized and scheduled by the Management Office. Clean-up The moving company and the Tenant will be responsible for leaving the Building and premises clean by removing all cartons and other trash generated in the move. If help is needed in the clean-up, Building personnel can be provided on a time and material cost/plus basis. Property Damage Any and all damage to the Building, elevator areas, doors, corridors, tenant spaces, or grounds which the Tenant, moving company or its employees or agents cause will be the responsibility of the Tenant. Required repairs will be accomplished by the Landlord with the expense billed to the responsible Tenant. Policies and Procedures: Smoking In compliance with the City and County of Denver Clean Air Act, Colorado Plaza Towers is a non-smoking building. All public and Tenant areas within and outside the Building are non-smoking areas, including the Lobby, restrooms and stairwells. There is, however, a designated smoking area located outside on the Plaza. This area is available 24 hours a day for Tenant use. Smoking within a tenant suite is not allowed. Policies and Procedures: Tenant Alterations REMODELING/REDECORATING It is imperative that Building Management is are aware of Tenant plans for remodeling and redecorating therefore, the Management Team will assist in the coordination of the remodeling or redecorating of Tenant suites through every phase of construction. During the initial phases, they will meet with the Tenant to find out exact requirements. Depending on the scope of the work, we may require working drawings and/or written specifications of the work prepared. When the specifications are complete, bids from outside contractors will be obtained. A formal proposal should then be prepared for the project. Upon Tenant approval of the proposal, the work will be coordinated and monitored by the Building’s Management through its completion. Remodeling/redecorating services available through the Building, either to be provided by the Building staff or outside contractors, are listed below. All remodeling of Tenant spaces must be coordinated through the Management Office. Some of the most frequently requested services are: Carpentry Carpeting/Tile Ceilings Electrical Heating, Ventilating and Air Conditioning (HVAC) Marble and Ceramic Tile Painting Plumbing Security: Building Access Access to the building is available 24 hours per day, seven days per week. Regular operating hours are from 6:00am to 6:00pm, Monday through Friday, and 8:00am to 12:00pm on Saturdays. Entry to the building at times other than 6:00am to 6:00pm, Monday through Friday requires a valid access card to gain admittance to the building and to allow elevator access to your floor (where applicable). Tenant Access System Cards To receive access system cards, please contact the Tenant Services Specialist in the Management Office. Lost or stolen access cards should be reported immediately to the Management Office. There will be a $25.00 charge for cards that are lost, stolen or damaged. When an employee is terminated, his/her access system card should be collected and returned to the Management Office immediately along with a copy of the form requesting a deactivation of the card. In addition, it is the Tenant’s responsibility to collect the keys and cards of all terminated employees in order to maintain the security of the suite. If replacement locks are needed because keys have not been collected from the individual(s) who have left the tenant’s company, there will be a charge for this service. Non-Building Employees After normal business hours, all visitor(s) must check in with the security officer on duty at the lobby desk to gain admittance to the Building. If the visitor(s) is accompanied by a Colorado Plaza Towers Tenant, the Tenant must record his/her guest in the security after-hours logbook. If an unaccompanied individual requests admittance to the Building, the security attendant on duty will telephone the Tenant’s office for approval. If guests/clients/visitors are to be permitted access to a tenant’s suite, the Management Office must receive notification in writing twenty-four hours in advance listing the individual’s name, company affiliation, purpose of visit, and approximate time of arrival. The Tenant is responsible for leaving a key for such individual(s). Visitors will be required to sign the logbook when they enter, as well as when they leave the Building. Security: Crimes In Progress Remember during any crime, personal safety comes first. If you see a crime taking place, follow these guidelines: Never put yourself in potential danger. Report the crime to either the Management Office, the Denver Police or a Security Officer, whichever is quicker. Get a good look at the suspect. Try to remember height, weight, age, sex, race and any distinguishing features. Do NOT try to apprehend the suspect. The person may be carrying a weapon. Do NOT follow the suspect outside of the building. If someone was injured during the crime, call 911 and request an ambulance. Security: Deliveries Dock Access, Freight Access, Insurance Requirements, Rules & Regulations Access to the loading dock is available 24 hours a day with 24 hours advanced notice and can be scheduled through the Management Office by email or telephone (303) 292-2300. For deliveries or dock access to be approved, please ensure that the service provider performing the work has a Certificate of Insurance on file that is in compliance with the appropriate Insurance requirements. Colorado Plaza Towers also has Building Rules and Regulations established for deliveries and work being performed in the Building. After hours, the loading dock can be accessed with the aid of security at the 633 Lobby desk, who must open the door into the dock. Any deliveries that cannot easily be carried in through the lobby doors must be delivered through the dock. No major items may be delivered between the hours of 7:00am and 10:30am, Monday - Friday. It is preferable that larger deliveries be scheduled later in the afternoon after 3pm, and coordinated with the Management Office. Large scale construction deliveries (i.e. drywall, metal studs, ductwork, etc.) need to be pre-approved by the Management Office and be delivered and unloaded before 7:00am, or special arrangements must be made with the Management Office. Tenants are responsible for any damage caused by their delivery personnel. For this reason, the Management Office requests your cooperation in instructing all delivery personnel to use the freight elevator when using dollies or delivering large items. Any items that cannot be easily carried in the passenger elevators must be transported onto the freight elevator. No deliveries using dollies or carts may be made through the lobby or across the lobby floor. The corridors on the tenant floors should be protected with masonite when moving large, heavy items. Security: Lost and Found Any individual finding lost item(s) should turn them in to the Management Office, Suite 1650 or the Security Desk in the main lobby of the 633 tower. You may also call us at (303) 292-2300 or visit the Management Office in Suite 1650 if you have lost any items. Security: Solicitation Solicitation is not permitted within the building. If you observe a solicitor, please notify the Building Management Office IMMEDIATELY at (303) 292-2300 and we will send appropriate personnel to have them escorted off the premises. Security: Special Keying All keys in the Building are included in a Building Master Key system. This key system is necessary so that the building staff has access to all areas in the event of an emergency. For this reason, we require that no locks be changed or additional locks be added to any door within your suite. If additional lock work for your suite is necessary, this service must be coordinated through the Management Office. There is a charge to re-key locks. Click here to see the Service Costs section of this handbook. As standard Building policy, all suites are re-keyed when new tenants move in. This helps ensure the security of that space for the new tenant. If you would like additional keys to any of the locks in your suite, they can be obtained through the Management Office with proper approval. Copies of keys can be obtained at a minimal cost of $3.50 each. In response to a Tenant’s internal security needs, we can provide additional services that include: Keying individual offices Re-keying the entire suite Coordinate with an outside contractor to install a security system throughout the space Security: Tenant Security Responsibilities Remember that the best way to improve security is for everyone to take an active role - just as you would in the neighborhood where you live. Make sure all doors to your offices are locked and secured at the close of your business day. This is extremely important on the weekends. Do not hesitate to report any suspicious or disorderly individuals to the Management Office at (303) 292-2300. Building Security will escort them from the Building. Solicitation is not permitted in the Building, and any individual who enters your offices for this purpose should be immediately reported to the Management Office. The Building Staff will escort them from the Building. Immediately inform the Management Office of any Building keys or access cards which are lost or stolen; including keys to your suite or storerooms. Each employee requiring after hours or weekend access will need to be issued a Building Access Card. Click here to see the Access Card and Parking Procedures section. Purses and or other valuables should never be visible from outside your suite, and it is suggested that they be kept in a secure place. Security: Theft Should you suspect that your office has been burglarized, or if items are found to be missing, contact the Management Office and the Denver Police Department. Our Security Officers submit a written report of these incidents to the Management Office immediately following investigation of the matter. In the meantime, try to avoid disturbing anything in areas which you feel might have been affected by an intruder. Security: Vendor/Contractor Access There may be special instances where vendors or contractors need to perform work in a Tenant’s suite during non-business hours. In such instances we request that you provide written notification to the Management Office stating the name(s) of the individual(s) and the company, the date they will be arriving and the approximate time. A brief description of the work to be done should also be included. All vendors and contractors must provide the Management Office with an approved Certificate of Insurance before work in the building may begin. Any vendor or contractor requesting that a portion of the building’s life safety system be disabled to perform their work will need to coordinate it through the Management Office. Services: Building Signage & Directory Tenant Signage Tenants are initially provided building standard suite signage and elevator directory strips. All initial and subsequent signage changes must be approved and coordinated through the Management Office. Directory Identification An electronic directory is located in the main lobby. Tenants must notify the Management Office, in writing, of the proper listing of the firm name. Additions or corrections to the Tenant listing must be submitted in writing to the Management Office. Services: Cleaning General Services Colorado Plaza Towers provides janitorial service Monday through Friday after business hours, excluding holidays. The following are services regularly provided by our Night Cleaning staff: All wastebaskets and other waste receptacles emptied daily All horizontal surfaces that can be reached without a ladder are dusted daily Drinking fountains and sinks are cleaned, polished, and sanitized daily Carpeted areas are vacuumed Non-carpeted areas are swept Hard surfaced floors are spot damp mopped daily Resilient tile floors are machine stripped and sanitized, periodically scheduled All restrooms are cleaned, sanitized, and regularly stocked with paper products daily If boxes or stacks of paper within tenant spaces are to be thrown away by the janitorial staff, the Management Office can provide special tags to identify these items for removal. Please contact the Management Office if you would like a quantity of these tags. Any large boxes or items that need to be disposed of during the day can be handled by the Day Porter. Under no circumstances should boxes or trash be placed in the hallway; as this is a fire hazard. Special Services If you have any special cleaning requirements please contact the Management Office. Some items will incur an additional cost. Individual space temperature control is achieved by means of electrical or pneumatic thermostats mounted in the tenant space that controls one of the eleven perimeter heating zones on the floor. Additional heating during occupied hours and pre-occupancy hours when outside air temperatures are below 32 degrees (or based on current weather conditions) is achieved by use of steam purchased from the local utility company. Steam is used to heat reclaimed condensate water from the building through the use of a heat exchanger. Water heated through a heat exchanger is supplied to the five main air handling systems located in the basement, 17th floor and 31st floor where the water is pumped through a set of water coils inside the air handler. Ventilation air is then pushed through this coil and distributed to tenant spaces depending on the time of day and the need for heat on the floors. Cooling is achieved through the use of either one 450-ton flat plate heat exchanger or two 950-ton centrifugal chillers, depending on outside air temperature and current weather conditions. This provides chilled water that is pumped through several water coils on the five main air handling systems located in the basement, 17th floor and 31st floor. Ventilation air is pushed through these coils to temper the ventilation air as needed. Ventilation is achieved by use of nine building supply fans and five return fans located in the mechanical areas in the basement, 17th floor and 31st floor. These fans provide continual air circulation throughout the building during business hours. Ventilation air is tempered by use of either outside air, building return air, or chilled water from the building’s flat plate heat exchanger or chillers depending on the season (winter or summer) and outside air temperature and current weather conditions. After the air is tempered it is distributed to all floors via ductwork located on each floor. Temperature in the tenant spaces is regulated by the use of variable air volume boxes, which increase or decrease airflow to the space depending on the temperature desired for that respective space. General Service Heating and air conditioning are provided in season Monday through Friday, 6:00am to 6:00pm, and 7:00am to 12:00pm on Saturday. After hours service can be arranged for an additional charge. Contact the Management Office for rate information. Special Services Heating and air conditioning are not regularly provided on Sundays, holidays, or after normal business hours. If you would like heating or cooling service at these times, please send a written request notice to the Management Office at least 48 hours in advance to schedule this service. Special air conditioning or ventilation may be needed for Tenants’ computer facilities, conference rooms, or personnel-intensive staff areas. We can aid you with the design and installation of additional vents or equipment to meet special needs. Please contact the Management Office for details concerning the requirements for this service. Energy Management In an effort to reduce the Building's single largest operating expense, energy costs, Building Management has implemented an energy maintenance system, thereby reducing energy consumption and the resulting escalatable operating expenses. The building is heated as needed, however, a lower temperature is maintained in the evenings and on weekends with a night set back. Interior lights are on timers and exterior lights are on timers and/or photocells. You can help reduce the cost of electricity by ensuring that all calculators, radios, computers, copy machines, coffee machines, and other heavy duty office equipment are turned off each evening. The night cleaning staff has been instructed to use only minimal electricity and to turn off all lights when they are finished cleaning any given area. Services: Mail Service Postal services are limited to sorting and delivering mail. There is a U.S. Postal mail chute located in the lobby mail area. Collection times are Monday through Friday: 1:15pm and 4:45pm, and Saturday at 1:15pm only. There is also an overnight FedEx drop box for tenant use, located in the mail area of the Lobby. The overnight FedEx that has placed a drop box in this area is listed below, along with their scheduled daily pick up time. Federal Express: Collection time Monday through Friday at 6:30pm The Management Office requests that couriers who bring mail or express mail abide by the Building’s delivery procedures. Mailbags and large package deliveries should not be brought through the main lobby. The loading dock entrance should be used for these deliveries. Delivery personnel must also use the freight elevator for deliveries requiring the use of a dolly or cart. Some tenants of the Building have couriers who bring mail or express mail to their offices at times other than regular delivery times. If after hour deliveries are to be made, they must be coordinated through the Management Office or lobby desk. Delivery personnel must be authorized to enter the building by the appropriate tenant contact with prior written notification to the Management Office. Services: Miscellaneous Services Colorado Plaza Towers also offers many amenities and special services for our Tenants. Please call the Management Office for further information regarding any of these services. Storage The Building contains a number of storage units primarily in the basement level of the building. These storage units are available for tenant use (subject to availability) at market lease rates. Please contact the Management Office for additional information. Window Washing Each tenant space is provided both interior and exterior window washing three times per year. In order for your windows to be cleaned properly, the Management Office asks that you keep the interior windowsills free of books, plants, and other items that might block access for the window cleaners. Building Management will advise you of the window washing schedule. Exterminating Occasionally there are problems associated with insect pests. Colorado Plaza Towers employs an exterminator that services the public building space. If you need an exterminator to visit your suite, please contact the Management Office to schedule this service. You should be prepared to give the exact location of the problem. To assist with pest control at the Building, please store food in glass or metal containers with seals. Services: Recycling Colorado Plaza Towers is a strong supporter of recycling efforts. We can assist you should you want desk side recycling containers for each person in the office. Items that can be put in the container are: White Papers: copy, letterhead, computer Colored papers: pastels only, carbonless Envelopes: empty white & pastel, plastic windows Direct mailing: opened, no adhesives Coated papers: fax, brochure, advertising File folders: manila or pastel only *Newspapers: separate bin If you have questions regarding the recycling of a particular material, please contact the Management Office. Desk side recycling containers are to be emptied by the Tenant into the larger recycling container within each Tenant’s suite which is provided by Building Management. The cleaning staff will empty the large containers as needed on Monday, Wednesday and/or Friday. It is imperative that the recycling containers contain only the categories described above. If you forget to sign up we will have a recyclable electronic drop location on the dock. Toma West is committed to environmental responsibility and sustainability. Services: Telecommunication Electrical/Telephone, CRT Line Installation Access to space occupied by other Tenants is sometimes required for the installation of electrical and telephone floor outlets and CRT conduits. For health and safety reasons, we require any above-ceiling work to be coordinated through the Building Engineers. Work of this nature is usually scheduled in the evening or on weekends so as not to disrupt the Tenants during business hours. Services: Service Requests For your convenience this handbook includes a work order system known as the Electronic Tenant Service Request System. Please use this system to submit routine maintenance requests directly to the engineering department, track the status of previously submitted requests, download important documents and to communicate with the Management Office. Completing a Service Request Form After logging in, click on the "Electronic Maintenance Request Form" Link. Users will be taken to a service request form. Step One- Confirm or complete all contact information. Step Two- Choose the nature or type of request being submitted. Step Three- If applicable provides details of the contractor to be used. Step Four - Review all information thoroughly. Click submit. You will receive conformation via e-mail that your request was submitted and received by Building Management. Updating User Information Personalized user information is used to auto-fill the Electronic Maintenance Request Form for quick and easy submission. In addition, accurate contact information will assist the management staff in expediting all maintenance requests. Each user should check regularly to ensure that accurate information is on file. Electronic Maintenance Request Log This feature allows users to track and monitor all service requests they have submitted through the Electronic Tenant® Service Request System. Service requests are sorted by month and will have the current month displayed upon entry. Miscellaneous Forms Here users can download and print various administrative forms, reports and documents. In order to access the forms and documents contained in this section, users must have Adobe Acrobat Reader 5.0 or higher installed on their computers.
