(Read this Product Disclosure Sheet before you decide to take out the CoPilot Personal Accident Insurance. Be sure to
also read the general terms and conditions.)
1. What is this product about?
CoPilot Personal Accident Insurance is a yearly policy that provides compensation in the event of accidental death and permanent
disablement whilst travelling in the private car owned by the Insured Person.

The Sum Insured amount is RM30,000 per Insured Person and the maximum sum payable for any one period of insurance shall
not exceed RM150,000.

5. What are some of the key terms and conditions that I should be aware of?
•
Cash before Cover - It is a fundamental and absolute special condition of this contract of insurance that the premium due
must be paid and received by Us before insurance cover is effective.
•
Claims Procedure - Notice of bodily injury on which the claim may be based on and which is covered by this policy, must
be given in writing to Us within thirty (30) days after the occurrence.
•
Automatic Termination – The insurance of the Insured Person shall automatically terminate on the earliest happening of
the following events:
(a) on the death of an Insured Person; or
(b) in the event a total of 100% is paid to the Insured Person during the period of this Policy, all coverage to that Insured
Person shall immediately cease to be in force.
(c) When the Insured Person attain the age of eighty (80)

What are the major exclusions under this policy?
This policy does not cover death or injury caused by the following events:
• War, Civil War, Hijacking, Terrorism
• AIDS and related diseases
• Pregnancy
• Provoked Murder or Assault;
• Travelling as an Aircraft Crew
• Aerial Activities, Martial Arts, Racing
• Radiation, Nuclear Weapons Material
• To any person while the Named Vehicle is used for hire, racing, road rally, pace making, speed-testing or used for any purpose
in connection with motor trade;
• To the driver if such driver does not hold a valid driver’s license to drive or is not qualified for holding or obtaining such a valid
driver’s license;
• While the Named Vehicle is used for illegal business pursuit as an unlicensed common carrier:
• Any person below the age of 5 and above the age of 80.

7. Can I cancel my policy?
You may cancel the Policy at any time by written notice of cancellation to Us and provided no claim is made under the Policy, You
will be entitled to a return of premium subject to our short period rates for the period of the Policy.
8. What do I need to do if there are changes to my contact details?
It is important that you inform us of any change in your personal details (including that of the nominee and/or trustee) to ensure
that all correspondence reach you in a timely manner. You can write in / fax us at the address / fax number below.

Pursuant to Paragraph 5 of Schedule 9 of the Financial Services Act 2013, if you are applying for this insurance wholly for purposes
unrelated to your trade, business or profession, you have a duty to take reasonable care not to make a misrepresentation in answering
the questions in the Proposal Form (or when you apply for this insurance). You must answer the questions fully and accurately.
Failure to take reasonable care in answering the questions may result in avoidance of your contract of insurance, refusal or reduction
of your claim(s), change of terms or termination of your contract of insurance.
The above duty of disclosure shall continue until the time your contract of insurance is entered into, varied or renewed with us.
In addition to answering the questions in the proposal form (or when you apply for this insurance), you are required to disclose any
other matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied.
You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed
with us any of the information given in the Proposal Form ( or when you applied for this insurance) is inaccurate or has changed.
YOU ARE ADVISED TO NOTE THE SCALE OF BENEFITS FOR DEATH AND PERMANENT DISABLEMENT IN YOUR INSURANCE POLICY.
YOU MUST NOMINATE A NOMINEE AND ENSURE THAT YOUR NOMINEE IS AWARE OF THE PERSONAL ACCIDENT POLICY THAT YOU
HAVE PURCHASED. YOU SHOULD READ AND UNDERSTAND THE INSURANCE POLICY AND DISCUSS WITH THE AGENT OR CONTACT

GOODS AND SERVICES TAX ('GST')
Please be informed that the Goods and Services Tax ('GST') will be implemented by the Government of Malaysia with effect from
1st April 2015 at a rate of six (6) per centum.
Zurich Insurance Malaysia Berhad reserves the right to collect from you an amount equivalent to the GST payable on the applicable
premium for the policy period, or in the event that the policy period commences before but expires after 1st April 2015, to collect
from you an amount equivalent to the GST payable on the applicable premium calculated from 1st April 2015 on a pro-rated basis.
Your obligation to pay GST shall form part of the Terms and Conditions in your insurance policy.
