CONDITIONS:
1. The general conditions of contract will apply to this transaction. This form, properly filled out,
constitutes the basis of agreement to supply the goods/services shown at the prices and within the
delivery period stated overleaf, if the bidder is successful. For quotes with a total value of over USD 10,000
and/or if there is a down payment, a specific contract will be signed between seller and DOT. If the down
payment exceeds USD 2000, a bank guarantee for the same value should be requested from the seller.
2. The offer shall remain firm for 30 days from the closing date unless otherwise stipulated by the seller.
3. Payment for goods/services shall be made by cheque within 10 working days after delivery of
goods/completion of services provided that DOT has received an appropriate original invoice from seller
and delivery vouchers duly signed by representatives of both parties.
4. DOT is not bound to accept the lowest or any other offer and reserves the right to accept any offers in
part or in whole.
5. Samples of offers when required will be provided free and before the closing date of the quotation. If
not destroyed during tests they will, upon request, be returned at the seller's expense, or may be collected
by the seller.

6. Making a payment to any employee as an inducement or any canvassing to enable you win this bid will
result in automatic disqualification from participating in this tender or any other tender.
7. Quotations with missing information will be disqualified from participating in this tender.
8. If you were not contacted after 10 working days from closing date, then be notified that your quotation
was not awarded.
INSTRUCTIONS:
1. Each quotation should state the below and addressed to the Procurement Person:
a. Supplier Name
b. Quotation Number
c. Description of Goods/Services with detailed specifications
d. Final Unit price which includes all cost for delivery, discount and VAT.
e. If VAT is applicable, please attach a copy of the VAT certificate or mention the VAT registration no. on
your quotation.

h. Payment Terms
i. Validity
2. Descriptive literature or samples of the item offered may be forwarded with the quotation.
3. An authorized representative of the seller must sign, stamp and date the quotation, and other relative
documents accompanying it.
4. If you do not wish to quote, please endorse the reason on this form and return it, otherwise your name
will be deleted from DOT list of suppliers for the items listed hereon.
