Holistic Care for People Living with Long Term Conditions Level 5 Module Supporting your patients, improving your knowledge This module is designed for both qualified and non-qualified staff working in primary, secondary and community care, who support patients with long term conditions. Conditions such as diabetes, dementia or COPD currently affect 20-30% of the UK’s adult population, and numbers are expected to increase substantially in the next decade1. This module provides a holistic overview of the area, equipping you with the knowledge, skills and attitude to confidently support and manage people living with chronic diseases, and to give them confidence to self-manage their condition. It is delivered by distance learning using comprehensive study materials and is accredited by The Open University. This module is compulsory for those taking the Diploma of Higher Education in Long Term Conditions.
