GENERAL INFORMATION
The Eurest Catering guide for Mallinckrodt Hazelwood is intended as suggestions for
your consideration. We are happy to assist you in planning for your special event. We
can create a menu specific to your themed event.
Catering Hours
Eurest provides Catering Monday-Friday, 7am-3pm. Exceptions can be made before or
after hours at an additional charge.
Linens, Tableware, & Utensils
All prices are based on the use of paper & plastic service ware. Tablecloths are
additional for $5 each. Cloth napkins, silverware, & china is an option at an additional
charge. This request must be made 72 hours in advance and will be at a contracted price
plus service charge.
Order Notification
All beverage orders require 24 hour notice, while luncheons & cakes require two
working days notice. To avoid charges to your department, a 24 hour cancellation notice
is required.
Sales Tax
All items are subject to a sales tax rate of 8.863%
Minimum Order
The minimum delivery amount is $20.
Required Information
For ordering accuracy purposes, all orders will be placed thru the on line website. For
Special events, contact Jason or Jessica at 4-3036.
Cancellation Policy
Any cancellation prior to the date of the event will not be charged. Any cancellation on
the day of the event are subject up to 100% of the charge.
