ATHLETE’S RULES OF CONDUCT

Please read this page in its entirety. By reading and signing this form, you, the athlete, and the parent
understand that you are allowing your child to participate in FAME VALLEY’S programs, related events and
activities. The undersigned acknowledges, appreciates and agrees to the following rules of conduct.
BUSINESS HOURS AND SCHEDULE
*Office business hours are 4 p.m. to 8 p.m. Monday through Friday (closed major holidays, dates TBA).
*Class schedule subject to change on a monthly basis. We will try and notify you of these changes but please
consult our website for current information: famecheer.com
*Classes with low enrollment may be cancelled, rescheduled or combined. Parents will be advised as soon as
possible.
GYM RULES
*Uniforms are not required to attend the tumbling classes and programs at FAME VALLEY, but we
recommend appropriate attire for your child’s safety. We will have required practice wear available for
purchase for the All Stars and recreational cheer programs.
*Parents are welcome to observe children in class from the waiting area, but we discourage criticism and
coaching from the sidelines. We believe that children build confidence through risk taking and concentration.
*Feel free to bring guests who may be interested in joining the gym to observe the class free of charge.
*FAME VALLEY will cancel class if the majority of students are more than 15 minutes late.
*FAME VALLEY is not responsible for lost or stolen articles. Please check the lost and found bin in the
reception area for missing items.
BILLING AND REFUND POLICY
*Schedule and Tuition: Our tuition payment schedule is month based. Because some months have more school
days, we charge the same for “long” months as we do for “short” ones (including holidays and school breaks).
There are no makeup classes for sessions that fall on traditional holidays.
*Tuition Payment: Monthly tuition is due in full on the first of each month. This payment will reserve your
child’s spot for the coming month. A late fee of $15.00 will be assessed on unpaid balances after the 10th of the
month. FAME VALLEY accepts cash, checks and all major credit cards. Automatic monthly payment can be
charged to a credit card upon request. There is a $30 charge for returned checks.
*Change/Withdrawal Policy: You may withdraw your child from any class or program at any time; however,
we request you notify FAME VALLEY of such intent as soon as possible so that we can plan accordingly. The
class or program must be dropped on or before the 1st of the month or you will be charged in full regardless of
attendance. For example, if notice is given in February, the child will be dropped from the class or program as
of March 1. February tuition will not be prorated. You may change your child’s schedule at any time provided
space is available.
*Refund & Makeup Classes: Tuition is not refundable. All credits will be applied toward future services. If
there are extenuating circumstances, please contact the office manager as soon as possible to temporarily
withdraw your child from the program and avoid unnecessary charges. Makeup classes may be arranged if
space is available. Parents must schedule makeup classes in advance to minimize disruption and ensure an
appropriate staff-to-student ratio. Parents must contact the office manager to obtain makeup class passes.
*Inform us if your child is allergic to any foods, latex or has any special needs, so that we can make sure that
we can fully accommodate you.
I will agree to comply with the program’s stated and customary terms and conditions for participation. In order
to properly protect my own child’s safety and that of fellow participants, I agree to follow these rules as well as
any others given to me by my coach, official, sponsor or any other agent representing FAME VALLEY.

All monthly tuition at FAME VALLEY is due on the first (1st) of each month. I understand that it is my
responsibility to make sure FAME VALLEY receives my full monthly tuition on or before the tenth (10th) day
of the month, or I will automatically be charged a $15.00 late fee.
