MONTHLY PERMIT PARKING AGREEMENT:
The following terms and conditions form a part of the monthly parking agreement with 134 High Street, LLC c/o Allpro Parking and are subject to change without notice.

Payment is due by the 1st business day of the month. Any outstanding accounts on the 5th day of the month will be charged a $15.00 late fee. 134 High Street, LLC reserves the
right to suspend or cancel the monthly agreement at any time and/or tow vehicle (at customer’s expense) if customer is in breach of terms and conditions herein. 134 High Street,
LLC also reserves the right to charge the daily rate for breach of terms.
134 High Street, LLC reserve the right to suspend or cancel the monthly agreement at any time, for any or no reason, upon thirty (30) days written or e-mailed notice to patron.
Forms of payment accepted: IN-STATE CHECK, MONEY ORDER, and MASTERCARD OR VISA. Any returned check will be assessed a $35.00 RETURNED CHECK FEE. 134
High Street, LLC reserves the right to decline acceptance of check if two or more returned checks have been received from any customer. To insure proper credit of payment, please
notify this office of any changes of address, phone number, etc. as soon as possible. MASTERCARD OR VISA payments will only be accepted on-line or by calling our main office
directly for instructions. DO NOT fax or mail account numbers.
Each monthly parking account will be assessed a one-time, non-refundable ACTIVATION FEE of ten dollars ($10.00) plus a ten dollar ($10.00) REFUNDABLE DEPOSIT on the
permit hangtag. The activation fee includes the cost of account set-up. If any hangtag is damaged through no fault of the user, it will be replaced free of charge if returned to 134
High Street, LLC. If hangtag is lost or stolen, a new one will be issued with the payment of a twenty-five dollar ($25.00) replacement charge. Upon cancellation hands-free hangtags
are to be returned to 134 High Street, LLC in good working order within 30 days and the user will be mailed the ten dollar ($10.00) refundable deposit.
Cancellation of your monthly parking agreement must be submitted in writing, by mail, e-mail, or fax, and sent to 134 High Street, LLC, at least ten (10) business days
prior to effective calendar month or you will be responsible for all applicable payments. Notification of cancellation by telephone will not be accepted. 134 High Street, LLC,
LLC does not issue refunds if cancellation takes place in the effective month.
Monthly Parking Permits are issued on a calendar month basis. NO REFUNDS, IN WHOLE OR IN PART, SHALL BE ISSUED AT ANY TIME. Parking rates and/or terms and
conditions may be subject to change, and such changes shall be posted in full view at the location 10-days prior. 134 High Street, LLC is not obligated to individually notify customers
of rate or terms and conditions changes.
Monthly parking does not guarantee the availability of a space; although every effort will be made to accommodate parking (it may be necessary to leave your ignition key). Should
no space be available, please see attendant for alternative parking.
Parking is permitted only in those spaces or areas that are NOT designated as reserved or restricted use unless you have paid for or qualify for same. If you park in a reserved space
not your own, or a restricted area for which you do not qualify, you may be issued a $50.00 parking violation and/or have your vehicle towed. Repeated occurrences will result in loss
of monthly parking privileges.
Monthly hangtags must be clearly displayed at all times from the rearview mirror on the front windshield of the vehicle, or as otherwise directed by 134 High Street, LLC, so that the
numbers are visible from outside the vehicle. Any vehicle not displaying a monthly hangtag will be subject to pay the posted daily rate. Transfer of a monthly hangtag to another
vehicle is prohibited unless that vehicle is listed on our records as an alternate. Transfer of a monthly hangtag to another person is also prohibited. Violation of this policy will result in
loss of monthly parking privileges.
Hangtag permits are only valid for the location for which they are designated. If the same hangtag number is found on more than one vehicle, or if a hangtag is used to admit more
than one vehicle at a time to the parking facility, or if any misuse is determined, that hangtag permit will automatically be deemed invalid without refund.
Parking patrons must park within one space as designated by the yellow lines. Parking patrons must also obey other location specific regulations such as compact car spaces, no
parking zones, handicapped designated spaces, electric vehicle charging spaces, etc. Vehicles are not permitted to block entrances, exits, fire lanes and other parked cars. Violators
will be issued violation tickets and/or be towed at owner’s expense.
This Agreement is for the licensed use of a parking space only and 134 High Street, LLC is not responsible for any loss or damage to the vehicle caused by fire, theft, collision or any
other cause to or damage to the vehicle or its contents.
Vehicles left unattended for 24 hours or more are subject to removal at the discretion of the Operator.
WARNING: VEHICLES SHOULD ALWAYS BE LOCKED, AND THE CONTENTS THEREOF SECURED IN THE TRUNK OR OTHER LOCKED STORAGE AREA OF THE VEHICLE.
I HAVE FULLY READ AND AGREE TO THE ABOVE MONTHLY PERMIT PARKING AGREEMENT. A CUSTOMER COPY OF THE TERMS AND CONDITIONS IS AT TACHED HERETO.

PROGRAM DESCRIPTION

What it comes with:
One Parking Account per Group
Carpool groups only need to pay for one parking tag every month, which they can then split the cost of between the members.
Carpool Only Spaces
We’ll give you special carpool parking tags that let you park in convenient “carpool only” spaces in the lot or garage.
2-for-1 Parking Tags
Sometimes you and your carpool partner might not be working at the same time. We’ll give you an extra parking tag for free to make carpooling easier.
No more having to track your partner down to get a tag when you’re the only one heading to work that day!
Please Note: Only one parking tag at a time can access the lot. If the first tag is already inside, the second tag will not be able to enter the gates. This
is done to prevent abuse of the system.
Enrollment in our Parking Validation Program
Sometimes you have to drive separately. We’ll put you on a list that lets you park your car for free 2 times per month on days when you can’t carpool but
work at the same time as your carpool partners. Each employee gets 2 validations, meaning your group can drive separately for free at least 4 times per
month!
Enrollment in our Guaranteed Ride Home Program
What if you carpool and the driver has to leave work early? What if your child is sick at school or you have to stay at work late? What if you don’t have a
car with you and an emergency occurs? Won’t you be stranded? Well, no. Each carpool member will be enrolled in our Guaranteed Ride Home
Program, which provides you with a free taxi ride to wherever you need to be when the unexpected occurs.
Membership to GO BNMC
Members receive access to all that GO BNMC has to offer, including VIP treatment at BNMC events and happy hours.
ELIGIBILITY AND PROGRAM RULES



Each registered group member must work at least 3 days per week on the Buffalo Niagara Medical Campus.
The primary contact for the carpool group must register all members associated with the group, and all vehicles associated with the group.
At least one group member must have / sign up for a BNMC parking tag / account.
