Dear Friend of HJI:
We are excited to be planning the 4th annual golf tournament for Hayden's Journey of Inspiration
Foundation (HJI). We are seeking tournament sponsors of $100 (Bronze level), $250 (Silver level),
$500 (Gold level) and $1000 (Corporate level).
The $500 and $1000 sponsorships also include a team
for the tournament (a $300 value). We would also love to have your 4-man team be there to play in
the tournament.
HJI is a non-profit foundation providing housing assistance for families of pediatric cancer patients
during their pursuit of potentially life-saving stem cell transplants at Duke University Medical Center.
One very special girl, Hayden Zavareei, who successfully made the same journey with her family,
founded HJI. HJI currently has three renovated and furnished apartments in Durham for use by
families who must relocate for treatments at Duke. To learn more, please visit our website at

The 4th annual HJI Golf Tournament will be held on Saturday, May 9, 2015 at Timberlake Golf Club in
Clinton, NC. Last years tournament had 108 golfers and raised around $21,000 for the HJI
Foundation. Proceeds of the tournament go directly toward supporting the families who must make
the journey to Duke for treatment.
Enclosed is a Sponsor and Team Registration Form. All sponsoring businesses will be listed on the
HJI sponsors board the day of the tournament. The corporate sponsors will also receive an individual

Hayden Zavareei, daughter of Natalie and Hassan Zavareei, and granddaughter of Danny and
Vicki Britt is now almost 17 years old and a leukemia survivor. She and her family have established the
HJI Foundation, whose purpose is to offer free, furnished apartments to families with children receiving
a stem cell transplant at Duke Medical Center where Hayden had a stem cell transplant that saved her
life at 6 years old. Currently, HJI has 3 furnished apartments housing families at a cost of around

We are looking for local families and businesses interested in sponsoring or entering a 4-person
team in the tournament. All sponsors will be listed on a sponsor board the day of the tournament and in
