have through me, against Hoffman’s Hoops Academy

for any injuries that may incur arising out of in any way
at the school/gym location during the months of June
and July, 2015. I further understand and acknowledge
that neither Hoffman’s Hoops Academy nor the
school/gym location shall have any responsibility for
lost, damaged, or stolen property. I further agree that I
have read and understand the right to photograph or
video my dependent and use the photo and/or other
digital reproduction of him/her or other reproduction of
his/her physical likeness for publication processes,
whether electronic, print, digital or electronic publishing
via the Internet. I also agree that my child may be
transported by bus and/or camp vehicle to an off-site
gymnasium or for emergency medical treatment. The
undersigned further expressly agrees that the attached
waiver and assumption of risks agreement is intended to
be as broad and inclusive as is permitted by law and that
if any portion thereof is held invalid, it is agreed that the
balance shall, notwithstanding, continue in full legal
force and effect.

2015 Hoffman’s Hoops Academy Basketball Training Camps: Pro Shooting Camp: Do you want to learn the best techniques in how to shoot? Are you tired of inconsistent results and frustrated by your results? This camp promises to provide you with the format to turn any player of any age into a more consistent, confident shooter. Each player will receive objective, measurable feedback through the use of video analysis and the top technology used by NBA and College teams. Sign up for both Pro Shooting Camps and receive a discount Top 25 Elite Middle School Skills Camp and Elite Youth Skills Camp: This camp is a must for any middle school player looking to take their game to the next level. This camp will be 70% skill work, 15% film study and 15% small-‐sided games. Players will go thru camp as if they were in college or the pros. Learn how elite players train and develop. Do not miss out on this great opportunity!! Top 25 Game Time Camp: This camp will combine skills into competitive games of 1 on 1 to 5 on 5. Players will develop their BASKETBALL IQ without the fear of making mistakes. Players will learn the concept of PLAYING PRESENT. This camp is sure to be the most fun and challenging camp they will attend this Summer! Super Shooting and Ball Handling This camp is design with the beginner in mind. Players will learn how to effectively dribble and shoot the basketball. The layers of proper shooting technique along with how to master the art of ball handling will be stressed throughout camp. Top 25 Post/Guard Specialty Camp This camp will focus on position specific skills. Players will be split into two groups and receive high-‐level instruction on how to dominate their position.
