HomePorts, Inc. has organized today’s Community Health Fair in an effort to strengthen local efforts to prevent disease and promote wellness for all ages in Kent County. We are also pleased to provide this expanded, updated Resource Guide and information about services available in Kent County. 1. HomePorts, Inc. HomePorts is now in its seventh year helping older adults in greater Kent County live independently and safely in their own homes. We are a non-profit, grassroots membership organization for those 55 and older. Over 100 seniors are members of HomePorts, benefiting from a cadre of over 50 volunteers who provide rides, home visits, and errands for members. Referrals are made to reliable service providers in the fields of transportation, interior & exterior home maintenance, home care, personal assistance & trouble shooting, pet services, and more. All vendors used are interviewed and approved by a review committee and are local business persons. Financial assistance is available to those on a limited income. HomePorts’ members also support and promote community services and encourage access to available social, educational, and cultural activities. A quarterly educational program features presentations on relevant topics, and social events are scheduled regularly. Many older adults believe they “are not ready yet” for an organization like HomePorts, but its adherents praise its operations. HomePorts is modeled after similar organizations operating successfully in other regions of the country, cited by experts as the wave of the future. Kent County offers many attractions to new retirees looking for a comfortable and senior-friendly place to live. It may be the smallest county in Maryland in terms of population but it’s the third largest, per capita, in the number of seniors, and a trend-setter in aging in place. If you’re wondering if you should get tested for diabetes or prediabetes, it’s helpful to review a checklist of common risk factors. If you’re over 45, you should ask your doctor if you should be tested for diabetes or pre-diabetes, even if the other risk factors don’t apply to you. For Overall Health: Bone Mass Index (BMI), including Bone Density About two of every three adults are now overweight or obese, and being obese puts you at higher risk for health problems such as diabetes, heart disease, high blood pressure and stroke. If you have a body mass index (BMI) higher than 30 it can lead to many chronic conditions and diseases, which is why it’s helpful to know your BMI. A body composition analysis will provide information that includes muscle and body fat percentages, and will include bone density levels too. (Please note that dehydration as well as large food intake prior to test may affect results.) Low bone density can lead to osteoporosis, which can lead to serious bone fractures. There is no pain associated with this condition - you could have osteoporosis or be at risk for it without even realizing it. A screening is recommended for women over the age of 30 and men over the age of 55 years old. Heart Health: Blood Pressure Screenings (BPs) High blood pressure greatly increases your risk of heart disease and stroke. Having high blood pressure often presents with no symptoms, thus regular screenings should be an important part of a healthy lifestyle to help detect risk factors in their earliest stages. Once risk factors are identified, appropriate lifestyle changes can be modified, such as diet or level of physical activity. There are a variety of medications available that can help to control high blood pressure too. Infection Prevention & Hand Hygiene A black light, which uses ultraviolet light, can show germs left on hands that have not been washed thoroughly. Test your hand hygiene skills and learn effective, easy ways to keep hands germ free! Lung Function Test (Spirometry/Lung Capacity) Spirometry is a common lung function test that measures how much and how quickly you can move air out of your lungs. For this test, you breathe into a mouthpiece attached to a recording device (spirometer). The information is collected by the spirometer and the results are reviewed by a physician, who will then determine if any necessary follow-up is required. Results will be mailed to the patient. 4. Health Screenings Stroke At-Risk Assessment More than 750,000 people suffer from stroke in the U.S. each year, but up to 50% of strokes could be prevented. In the case of stroke, many of the risk factors can be treated, modified or controlled. However, some risk factors for stroke cannot be changed. Stroke risk factors are medical conditions, behaviors or traits that make you more likely to develop a stroke. Having one or more risk factors does not mean that you definitely will have a stroke, only that you are more likely to do so. Take the at-risk stroke assessment to assess your risk of stroke. Lab Screenings & Tests: HDL & Total Cholesterol ($5 each) Total cholesterol levels are known to be related to Coronary Heart Disease (CHD). Your total blood cholesterol is a measure of LDL cholesterol, HDL cholesterol, and other lipid components. HDL levels are also important in assessing your risk of CHD because HDL cholesterol - the “good” cholesterol - protects you against heart disease by removing the LDL cholesterol out of your blood and keeping it from building up in the arteries. A small sample of blood will be drawn from your arm and your blood sample is then analyzed in a laboratory. You will receive a letter in the mail within 2 weeks with your results. Your healthcare provider will use your total cholesterol number when determining your risk for heart disease and how best to manage it. 5. Screening Guidelines for Women Check the guidelines listed here to find out about important screening tests for women. These guidelines are recommended by the U.S. Preventive Services Task Force. Keep in mind that these are guidelines only. Your doctor or nurse will personalize the timing of the screening tests you need based on many factors. Ask your doctor or nurse if you don’t understand why a certain test is recommended for you. Check with your insurance plan to find out which tests are covered. Insurance companies are required to cover many preventive services for women at no cost to you because of the Affordable Care Act. Test: Ages 18-39 Ages 40-49 Ages 50-64 Ages 65 + Blood Pressure Test All Age Groups: Get tested once a year if you have normal blood pressure (lower than 120/80). Get tested once a year if you have blood pressure between 120/80 and 139/89. Discuss treatment with your doctor or nurse if your BP is 140/90 or higher. Bone Mineral Density Test Discuss with your doctor or nurse if you are at risk of osteoporosis. Get this test at least once at age 65 or older. Talk to your doctor or nurse about repeat testing. Breast Cancer Screening If you have a family history of breast cancer, your healthcare provider may suggest screenings at an earlier age. Starting at age 50 get screened every 2 years. Get screened every 2 years through age 74; age 75 + ask you doctor. Cervical Cancer Screening (Pap test) Get a Pap test every 3 years if you are 21+ and have a cervix. If 30 + or older, get a Pap test and HPV test together every 5 years. Get a Pap test and HPV test together every 5 years if you have a cervix. Get a Pap test and HPV test together every 5 years if you have a cervix. As your doctor or nurse if you need a Pap test. Chlamydia Test Get tested yearly thru age 24 if you are sexually active or pregnant. Age 25 + get tested if you at increased risk, pregnant or not pregnant. Get tested if you are sexually active, and at increased risk, pregnant or not pregnant. 6. Ages 50 - 65 +: Get tested if you are sexually active, and at increased risk. Screening Guidelines for Women Test: Cholesterol Test Ages 18-39 Starting at age 20 get tested regularly if at increased risk for heart disease. Colorectal Cancer Screening (using fecal occult blood testing, sigmoidoscopy, or colonoscopy) Get screened for colorectal cancer through age 75. Gonorrhea Test Ages 18 - 49: Get tested for gonorrhea if you are sexually active and at increased risk, pregnant or not pregnant. The Federal Trade Commission says there are many variations but that these schemes are getting bolder. • Don’t pay to collect sweepstakes winnings • Hold onto your money • Look-alikes aren’t the real thing • Phone numbers can deceive The best tool against elder financial abuse is prevention. • Do not ever give your personal information to someone on the phone • Do not buy anything from a door-to-door salesman; do not answer the door if you don’t have to • No legitimate purchase or investment decision needs to be done immediately • Credentials and referrals matter when dealing with professionals. When possible, go local • Just because Medicare or Medicaid agree to pay for something doesn’t mean the service or item is legitimate • When in doubt, get a second opinion - an ethical salesperson welcomes a second opinion • And finally, if it doesn’t feel right, trust your instinct! According to the AAA, senior drivers are at a higher risk of having a serious collision per mile driven than any other age group except for those under age 25. Drivers in their late 70s have about the same number of injury-involved crashes per mile driven as drivers in their early 20s. Hearing Loss May is Better Speech and Hearing Month Impaired hearing is one of the top three chronic diseases in the country. An estimated 28 million Americans have hearing loss or “presbycusis”, including one-third of those aged 65 -74 and over half who are 85 and older. Fewer than 25% of those impaired use hearing aids, though presbycusis has been shown to decrease one’s quality of life and increase the incidence of depression in seniors. Presbycusis is treatable in most cases. Hearing impairment affects all of those who interact with the patient as well. As we get older, physical changes and health conditions - and sometimes the medications used to treat those conditions - make falls more likely. Falls are the leading cause of death by accidental injury for those over 65. Six in ten falls occur in the home and can lead to fear of falling, sedentary behavior, impaired function or lower quality of life. The Mayo Clinic offers six simple fall prevention strategies: 1. Make a fall-prevention plan and discuss it with your doctor • Make a list of your prescription and over-the-counter medications and supplements, or bring them with you to the appointment • If you’ve fallen before, write down the details, including when, where and how you fell • Be prepared to discuss your health conditions and how comfortable you are when you walk 2. Keep moving If you avoid physical activity because you're afraid it will make a fall more likely, tell your doctor. He or she may refer you to a physical therapist who can create an exercise program aimed at improving your balance, flexibility, muscle strength and gait. 3. Wear sensible shoes Consider changing your footwear as part of your fall-prevention plan Wear properly fitting, sturdy shoes with nonskid soles 4. Remove home hazards • Remove boxes, newspapers, electrical cords and phone cords from walkways • Move coffee tables, magazine racks and plant stands from high-traffic areas • Secure loose rugs with double-faced tape - or remove loose rugs • Use nonslip mats in your bathtub or shower 52. Use assistive devices • • • • • Hand rails for both sides of stairways Nonslip treads for bare-wood steps A raised toilet seat or one with armrests Grab bars for the shower or tub A sturdy plastic seat for the shower or tub, plus a hand-held shower nozzle for bathing while sitting down If necessary, ask your doctor for a referral to an occupational therapist.
