Dear Huston Camper & Family,
Please confirm your van transportation details below - sign the attached permission slip and return it with your
Health Form and final payment at least 30 days before the session begins.
In the vehicle, campers are expected to wear seat belts at all times and listen to the driver. If someone other than the
parent/guardian will pick up your camper at the end of camp, please provide that person’s name below.
Parents/guardians are responsible for their campers’ safety at the pick-up and drop-off sites. The Camp Huston
transportation vehicle cell phone is (360) 348-1712. While being transported by Camp Huston, all passengers will be
seated wearing a safety belt. Passengers are not allowed to extend any object or their extremities out of the window
for any reason.
Please see the reverse side of this sheet for specific pick-up and drop-off times for your Session. The pick-up and
drop-off locations are listed below. Transportations costs $35 one way, and $50 round trip. If you haven’t sent the
transportation payment, please do so with the Health Form and Transportation Permission Slip.

If there are changes or an emergency that would affect the camper’s pick up or drop off time, or location, Camp
