TEEN KLINIC There are no scheduled appointments at Teen Klinic. Everyone is seen on a walk-in basis. Service is limited to the number of clients that can be seen during the clinic hours. Teen Klinic provides primary health care services to youth ages 1220 who are unable to receive these services elsewhere. The services are provided by counsellors, nurses, nurse practitioners and physicians. If you have never been to Klinic, you will be seen by a counsellor first, then, if appropriate, you will see a nurse, nurse practitioner, or physician. You will not need to see a counsellor at every visit. WHAT INFORMATION DO I NEED TO GIVE KLINIC? Teen Klinic is a time set aside to provide services specifically to youth. No one will judge you because of your age, gender, appearance, or sexual orientation. It is important to use your legal name, as it appears on your health card, when you come to Klinic. No nick names or slang please. It is important to provide us with a phone number that Klinic staff may call you at. Klinic staff will only contact you if your tests need to be repeated, if you need medication, if you have an infection, or if the call is medically necessary. WHO WILL KNOW THAT I CAME TO KLINIC? Services at Klinic are confidential. This means that your visits are just between you and your health care providers at Klinic. You do not need the consent of your parent or guardian. If you wish, you may bring a friend, parent, or guardian with you to Klinic. The only exception to Klinic's confidentiality policy would be in the case of a young person being at risk and in need of protection. Every person receiving services at Klinic has a health record. Whenever you see a Klinic service-provider, all relevant information regarding your care is documented to ensure quality services. Only Klinic staff directly involved in your care will access your chart records. Your Klinic chart records can be transferred to another health care provider at your request.
