JUNIOR COMMUNITY SPORT EXPO REGO FORM
The FUTURECLUBS Club Development Program will be holding a Junior Community Sports
Expo for primary school aged children to experience a number of junior sports.
Over the duration of the Expo groups of approximately 20 to 40 like aged participants will
take part in a number of drills and games delivered by coaches, parents and players from
various local sporting groups. The aim of the Expo is to give children the opportunity to try
a number of sports with their parent’s involvement and to provide information on what
sporting options are available in the community.

Time: 4:00pm to 7:00pm
Where: Karratha Leisureplex, Dampier Highway Karratha

Spaces are limited. Please use this registration form to reserve your child/children’s place
in the Expo activities. Parents are required to stay for the duration of the Expo and follow
their child/s progress through each sport.

This registration form must be completed and returned to Sam King – City of Karratha Club
Development Officer. Please do not return the nomination form to the school. Once your
registration form has been received you will be sent a confirmation email stating your
child/children have an allocated place in the Junior Community Sport Expo.
Prior to the Junior Community Sport Expo you will then be sent an email reminder. This
email will contain:

A Junior Community Sport Expo Map
o
You and Your Child’s/Children’s Junior Community Sport Expo Team Name and
Uniform (Shirt) Colour
If all available spaces have been filled, once your application is received you will be sent an
email to that affect.
We would like to accommodate all sports and registrations but unfortunately due to limited
resources we can only accept a set number of registrations. We will hopefully expand to
accommodate more sports and participants in the future.
