SUMMER BASKETBALL CAMP (Grades 3 – 8)
PARENTS RELEASE AND INDEMNITY AGREEMENT
I hereby authorize the staff of the Kearney High School Lady Bearcat
Basketball Camp to act for me according to their best judgment in any emergency
requiring medical attention, and I hereby waive and release Kearney High School
Lady Bearcat Basketball Camp and Kearney Public Schools and all their
employees and agents from all claims on account of any injuries or illness
sustained by my child while attending the above said camp.
