_____ I acknowledge that all uniforms and/or equipment issued are the property of the
Langley Eagles Pop Warner Association and that I am obligated to return it at the conclusion of
the current season or upon request.
_____I acknowledge that if I fail to return any uniforms and/or equipment that I will be
responsible for paying the replacement cost in its entirety.
_____I acknowledge that if any uniform and/or equipment is lost, stolen, or damaged (with the
exception of normal wear and tear), that I will be responsible for paying the replacement cost in
its entirety.
_____I acknowledge that no alterations, additions, or modifications may be made to uniforms
and/or equipment without express written consent by a Langley Eagles Pop Warner Association
board member and that doing so may result in termination of registration.
_____ I acknowledge that participants may not permit uniforms and/or equipment to be used
by another party or at a different location without the express written consent of a Langley
Eagles Pop Warner Association board member.
_____I give permission to have my child’s picture, video, or any media displayed on the
organization’s website and social media outlets.
_____I acknowledge that all FUNDRAISING IS MANDATORY and agree to participate fully in all
of the organization’s fundraising efforts or opt for the monetary buy-out.
_____I acknowledge that the registration costs are $200 per participant and that $100 of that
cost may be raised using the season’s initial fundraiser.
____ I acknowledge that as the parent/guardian of the participant that I will be called upon
during the season to volunteer to assist with activities such as concessions, field chain/marker
crew, mandatory play recording, etc.
____ I acknowledge that registration fees do not include costs associated with travel or
pre/post-season play and or competitions.
____ I acknowledge that all registration fees must be paid before any uniform and/or
equipment is issued to the participant.
____ I acknowledge that all registration fees are non-refundable upon receipt and that there
will be no exceptions.
____ I acknowledge that all registration paperwork must be completed and submitted prior to
August 1st and that failure to do so will result in the participant’s registration being voided and
their name being removed from the respective team’s roster until they are compliant.
