This form, all pictures and stapled business card must be submitted before 3:30 pm on
WEDNESDAY, MAY 27th, 2015 - - - ONLY to the Shorewood High School Security Office.

One guest per Shorewood Host student
Guests must be pre-approved prior to attendance at an event. Pre-approval process includes Approval Check Sheet on Security Office door.
All Guests must be under the age of 21 AND currently enrolled high school students in good standing at an accredited high school; OR a graduate of
Shorewood or Shorecrest only.
Everyone, including Host and Guest MUST bring photo identification to the event. Acceptable forms of identification include only:
Current high school ASB / ID card; OR State-issued driver's license; OR State-issued identification card.
Host and Guest must comply with all rules and expectations of Shorewood High School and the Shoreline School District. Failure to do so will result in
removal from the event, school discipline, and/or possible criminal charges. Shoreline School District 2013-2014 Statement of Student Responsibilities
