GENERAL INSTRUCTIONS:
1. Only those students who have passed the Intermediate / Class XII in April / May 2015 in the first attempt with a minimum
of 60% / 55% marks (as applicable for different courses) are eligible to apply.
2. No admission would be granted without the presence of the parent at the time of interview.
3. Read the prospectus of Loyola Academy carefully before filling up this application form.
4. Entrance Exam and Interview will be conducted before the student is admitted, and the result will be displayed on
Loyola Academy College office notice board.
5. Kindly refer to the hand-out supplied along with the application form for the date and time of the entrance exam, date of
the entrance exam result and the date and time of the interview.
6. All entries should be in the candidate’s own handwriting and in English, as per SSC / Class X memo, and the candidate
will be held personally responsible for any incorrect entry.
7. The college reserves the right to cancel the admission of the candidate at any stage, if and when it is detected that the
admission is against the rules, and is secured by giving wrong or incorrect information.
8. At the time of admission, the original Class X and Intermediate / XII mark sheets, original TC, Bona fide / Conduct,
Indemnity form and Caste certificate, if applicable, should be submitted within the stipulated time, failing which the
admission would be deemed invalid / cancelled.
9. Original certificates will not be returned till the completion of the course.
10. Ten photo copies (Xerox) of all certificates of the School and Intermediate / Class XII should be enclosed at the time of
admission. Students should keep enough Xerox / scanned copies of their original certificates with them for any future use.
11. Students should also submit photo copies (Xerox) of the Ration Card and the Aadhar Card.
12 Please ensure that there are no mistakes in the original certificates.

Candidates must carry their examination hall ticket with them for every session of the examination.
In case of loss, a duplicate will be provided on payment of a prescribed fee.
Candidates will not be allowed into the examination hall, 15 minutes after the commencement of examination.
No candidate will be permitted to leave the hall before the end of the session.
Each one should occupy the seat assigned to him/her in the hall, at least 10 minutes before the
commencement of the exam.
There should be no malpractice or attempt at malpractice. Therefore, do not take cell phones, organizers,
books, bits of notes or other printed or written material into the exam hall. Similarly, consultation of any sort
is prohibited.
All must keep strict silence in the Examination hall.
Candidates should sign their names in the exam attendance register in every session.
Question paper and Answer scripts should be handed over to the invigilator. They should not be left on the
table.
