I understand that unless I, my family member or my guest ("we") have sought prior approval from a doctor, we should
refrain from participating in any HKJC organised activity if I/family member/guest suffer(s) from a bone/joint/muscle
condition, allergy, high/low blood pressure, heart condition, fever, stroke, infectious disease, asthma, diabetes, epilepsy,
any other health issue not listed here or when under the influence of alcohol or drugs.
Inclement Weather Arrangement
Ordinance, which restricts our use of these data. The Notice is available on Form DP1, which may be obtained from the
Membership Services Department, 7/F, Jockey Club Headquarters, 1 Sports Road, Happy Valley, Hong Kong or from the
Club.

Members, Corporate Members and Clubhouse Members are eligible to join the Happy Valley Clubhouse Summer Odyssey
Programmes.
There will be no activities if a Tropical Cyclone Warning Signal No.8 or higher, Red Rainstorm or Black Rainstorm Warning
is hoisted.

NOTES
• Members’ children not officially registered with the Club will be subject to a guest charge.
• Members’ guests are permitted to take part in the Summer Odyssey Programmes but must join the same programme(s) as the
host Member(s) (except at the Beijing Clubhouse). Members have priority in all programmes. Guest charges will be applied.
Please note that all classes have minimum participation requirements to proceed. Confirmation of classes will be sent out by fax
or mail. Members who wish to cancel need to notify the Club prior to the dispatching of the confirmation letter.
• Registration forms received prior to 11am on 6 June 2015 will be placed on a waiting list and allocated a random queue number
from 9am on 7 June 2015.
• Members who wish to accompany their children at the visits should also provide their names on the application form.
