WAIVER/RELEASE FORM
Parental release
I, the undersigned, being the legal guardian of
the above-named child, do hereby grant permission for participation in all activities sponsored
by the Junior Field Hockey Recreation program
and Noon Sports Academy. I assume all risks and
hazards incidental to such participation and release the Mountain Lakes Junior Field Hockey
program, Noon Sports Academy, and its agents
and assigns from any liability and/or responsibility for any injuries sustained by my child or expense incurred there from while engaged in
or traveling to or from any activities of the
above-named organization. Further, I attest that
my child is in good health and able to participate
in the Mountain Lakes Junior Field Hockey
program and Noon Sports Academy. I agree to
notify my child’s coach if there is any medical
problem of which she should be aware. If my
child incurs an injury and I cannot be reached, I
give the coach my permission to seek emergency
medical care.

Noon is the Associate Athletic
Director and Head Field
Hockey Coach at Fairleigh
Dickinson University-Florham
Campus. The Devils finished
their most decorated season as
they advanced to the “Sweet
16” of the NCAA Tournament. Noon lead the
team to a Conference Championship in 2014,
ECAC Mid-Atlantic Finalists in 2012 and
Freedom Conference Finalists in 2011. Noon
was named Freedom Conference “Coach of
the Year” in 2014 and 2011. Noon also
coaches for the United States Field Hockey
Futures Program and is a Level I and Level II
USA Field Hockey Sanctioned Coach. She
serves on the NCAA Regional Ranking Committee, the NCAA All- American Selection
Committee, the NCAA Coach of the Year Selection Committee, and the ECAC Championship Selection Committee. Noon also
owns and directs Competitor’s Edge Field
Hockey Camps.
A standout field hockey player at Rutgers
University, Noon was a four time letter winner and was named First Team All-Big East
in 2003. She was also a three time AllAcademic player, a member of the National
Academic Team, Big East Offensive Player of
the Week, and a recipient of the field hockey
team’s “Mission Statement Award.” Noon
currently is a member of USA Field Hockey,
the NFHCA, and the NJFHA.
Noon Sports Academy was created to
provide more playing and learning opportunities for youth in the North Jersey area. Our goal is to teach fundamental field hockey skills in a positive
learning environment. Drills are specifically designed to give players more
touches on the ball, repetitions, and
small group coaching for quick improvement. Players will be divided by
age/skill levels and go through circuit
training and small game play.
This Academy will strive to give each
player the confidence and inspiration
to continue participating. We hope to
spread our love for the sport to the next
generation of field hockey players.
