The cost of blank tender document is Rs 500/- (Rs. Five Hundred Only) (Non
refundable). The cost of tender document is to be paid in cash or by demand draft of any
Nationalised Bank in the name of “The Registrar, Maharashtra University of Health Sciences,
Nashik”. To obtain the tender document please submit application on the letter head of the
firm. Tender document can be downloaded from University website www.muhs.ac.in. The
Tenderer who are downloading the tender document from the website shall pay the cost of
tender documents by demand draft of any Nationalised Bank at the time of submission of the
tender document. All other terms and conditions are mentioned in the tender document. The
University reserves the right to reject any/all tender.

The Detailed technical specifications of the requirements mentioned above are given in
the “ANNEXURE – I” appended to the tender.
2.
General Guidelines for Submission of Tender
Please read the following conditions carefully before you fill up and submit the
tender form.
2.1
Every bid is to be submitted in two parts, viz.,
(a) Technical Bid. (b) Commercial Bid.
2.2
Only the Commercial Bid will contain rates. The technical bid must not contain the
rates. If it does, then the whole bid will be summarily rejected.
2.3
Technical bid and commercial bids are to be submitted in two separate envelops
which may be called the inner envelops. The envelopes must be securely sealed.
2.4
These inner envelopes must be superscribed suitably mentioning the kind of bid
such as “Technical bid” or “Commercial bid”.

Technical bid and Commercial Bid envelopes should be put in a bigger
outerenvelope, the outer envelope should be superscribed suitably mentioning
“Tender for supply, installation & commissioning of Fax Machine due
on 09/04/2015”
2.6
The tender should be addressed to “Registrar, Maharashtra University of Health
Sciences, Vani Road, Mhasrul, Nashik - 422 004.”
2.7
In their own interest, the bidder should submit the tenders through their
representatives. Postal delays will not be considered.
2.8
Faxed and emailed bids will not accept.
3.
Documents to be enclosed with Technical Bid
(A) Authorisation letter
(1) The authorization letter on the letter head of the principal company allowing
their authorised Dealer/ distributor or representative to submit the tender quoting
the Tender No. & duly signed in ink should invariably be enclosed in the technical
bid.
(2) Certified-copy of Shop Act registration certificate of Company Registation
document, Central Sales Tax, VAT registration certificate, PAN No. & Central Sale
Tax, VAT clearance Certificate / Challan for the period ending Dec. 2013, must be
enclosed with the technical bid.
(B) ISO Certificate
If the manufacturer has ISO/ISI accreditation, the Photo copy of the same may be
enclosed in the technical bid envelope.
(C) Earnest Money Deposit (E.M.D)
The Technical Bid envelope must be
accompanied with a bank demand draft of
Rs.4,500/- (Rs. Four Thousand Five Hundred only) as Earnest Money Deposit
(E.M.D.) The E.M.D. may be in the form of crossed Account Payee Demand Draft
drawn on any nationalised bank payable at Nashik In favour of “Registrar, M.U.H.S.
Nashik” Tender without E.M.D. will be summarily rejected.
(D) Technical Information
The relevant product information, Company profile, Technical Specification sheets,
Technical Test results should be submitted along with Technical Bid. Failure to
submit this information could result in disqualification.

(E) Technical Compliance Report
Annexure-II
Technical Compliance Report as given in Annexure-II must be filled in and submitted
alongwith Technical Bid.
(F) Acceptance of Terms and Conditions of Tender
(1) Enclose the copy of tender document with TechnicalBid duly signed and stamped
at the bottom of each page as a token of acceptance of tender terms & conditions.
(2) Conditional offers are liable to be summarily rejected.
(G) Annexure –III (Check List)
Duly filled
in must be

(H) Indicate the names of reputed organistion where you have supplied the similar
equipments/goods.
4. Document to be enclosed with commercial Bid
Quotation Annexure-IV
The Commercial Bid must be in the Proforma given in appended with the tender.
The Commercial Bid must be signed & stamped by anauthorised signatory.
5. General Terms and Conditions
(A) Rates
The item rates and ACMC Charges rates must be quoted
Annexure-IV and Annexure –V
by
the
bidder
in
The rates of the item must be inclusive of all
taxes, LBT, levies, insurance, transportation of the material and installation &
commissioning and on site comprehensives warranty. The period of on site
comprehensive warranty for each equipment should be mentioned in Annexure –IV
successful bider should buy back the old Fax Machines (Make – Panasonic KXFT
910) as mentioned in the Annexure I. The buy back rate should be mentioned in
commercial bid i.e. Annexure IV clearly
(B) Validity of offer
The offer should be valid for the period of 4 Months (120 Days) from the date of
opening of the Commercial Bid.
(C) Transfer of Tender Document
The tender document is not transferable. Only the party who has purchased the
tender document shall be entitled to quote.

(D) Erasures/Alteration
The offer containing erasures or alteration will not be considered.
(E) Date & Time of submission of tender
The tender duly signed and securely sealed should reach to the MUHS, Nashik
(Purchase Section) on or before 09/04/2015 up to 3:00 p.m. As far as possible the
technical bid will be opened on the same day in the presence of attending tenderers.
If the tender is not opened on the due date, then the date and time of opening the
tender will be communicated separately.
(F) Short Listing of Vendor
The University will short list technically qualifying vendors and commercial bids of
those vendors will only be opened later. The venue, date and time for opening of
commercial bid will be intimated at appropriate time to the concerned bidders.
(G) Acceptance of tender
1) The University does not bind itself to accept the lowest or any other tender. The
University has reserved the right to reject any or all tenders with out assigning any
reason for the not acceptance.
2) Mandetory condition for commercial bid.
The University Consider lowest rate for monitors after deducting the buy back cost
of old Fax Machines. Plese knindly refer Annexure – IV.
(H) Agreement
Acceptance of Purchase Order by the successful tenderer is itself an agreement. A
separate agreement between Tenderer and M.U.H.S., Nashik on Hundred rupees
bond paper has to be executed and the same must be notarised from authorised
notary. The cost of bond paper and notary expenses will be borned by the tenderer.
(I) Security Deposit
EMD of successful tenderer will be Converted in to Security Deposited and valid for
warranty and ACMC period, from the date of installtion & commissioning. EMD/
Security deposite Amount will be returned back only after successful completion of
warranty and ACMC period from the date of installation and after due certification
by the User Department/University Engineer (Electrical Section) or the team
appointed by the University.

(J) Warranty
The bidder must give on site comprehensive warranty for Fax Machines including
all parts. The warranty maintenance period as per company norms will commence
from the date of Installation & commissioning and due certification by the User
Department/University Engineer (Electrical Section) or the team appointed by the
University.
(K) Terms of Payment
Payment will be made by RTGS/Electronic Clearing System on successful
installation and Commissioning & receipt of agreement and after due certification
by the User Department/ University Engineer Electrical Section or the team
appointed by
the University after satisfactory Installation and Commissioning of
Fax Machine.
(L) Delivery, Installation & Commissioning
The bidder must deliver the material within a Two week from the date of purchase
order. The bidder deliver all the material in one lot, in any case part supply will not
be accepted. Installation and commissioning must be completed immediately after
the supply of material. In case of delay in delivery, installation and commissioning,
the penalty at the rate of One Percent (1%) per weak of accepted value will be
charged and will be recovered from due payments or Security Deposite (S.D.)
(M) Inspection
The material to be supplied should be strictly as per the technical specification
mentioned in the tender and Purchase Order. The delivered material will be
inspected by the User Department/ Uinversity Engineer (Electrical Section) or the
team appointed by the University. The supplier will have to replace the rejected
material at their own cost immediately.
(N) Increase or Decrease in quantity
The quantity of items mentioned in the tender may be increased or decreased. The
supplier will not be entitled for any compensation for a decrease in the quantity. In
case of increase in the quantity items, the supplier must supply the items at the rates
mentioned in the Tender.
(O) Technical Conditions
(i) The terms and conditions of Warranty may be enclosed in the Technical Bid.
(ii) The terms and conditions of ACMC may be enclosed in the Technical Bid.

The tenderer shall inform about the method of support system evolved by
him during the warranty period, he should also mention the minimum anticipated
down time, etc. The University expects minimum response time of 4 hours and
maximum time shall be 24 hours
PENALTY
During warranty maintenance period if the down time for Fax Machine exceeds more
than 4 hours, then penalty at Rs.200/- respectively per day Per system will be charged
& recovered from payments due to the Contractor.
(iv) All sorts of manuals, drivers, connection cords required for installation,
commissioning and use should be provided with the Fax Machine.
(v) The supplier should replace all the damaged parts (if any), & any incompatibility
found during the installation will have to be rectified at their own cost.
(P) Failure to supply
In case the tenderer fails to supply the ordered material within the stipulated time
period. The University shall forfeit the E.M.D. amount.
(Q) Signature on Tender document
Tender document must be signed by the authorized Signatory or Proprietor.
(R) Refund of E.M.D.
The E.M.D. of disqualified tenderers will be refunded immediately after completion
of tender procedure.E.M.D.ofqualified tenderer will be refunded after receipt of
agreement & as per Clause 5(I) & 5(K) respectively and certification by concerned
authority.
(S) Governing Laws & Jurisdiction.
1) The decision of the Hon’ble Vice Chancellor will be treated as a final decision in
any leagal Complication arising in future.
2) No suit or any legal proceeding with regard to any matter arising in respect of this
tender shall lie in any court save the competent city civil court at Nashik.
