2015-2016 Enrollment Contract
Mountain View Montessori (“MVM”) agrees to enroll
(“Student”) for the 2015-2016 school year and to provide the program(s), educational and other services
as prescribed by said programs at the request of ____________________________________________
(“Responsible Party”) as Student’s parent(s), caretaker, guardian and/or financially responsible party.
For New Students Only: There is a 2 week Trial Period for Responsible Party, Student, and MVM to
make sure that this is a good choice for all. (In the event that you choose to withdraw your Student at the
end of the Trial Period, then the Registration Fee will be forfeited and this Enrollment Contract will be
null and void. However, in the event that your Student is dismissed by MVM at the end of the Trial
Period, then the Registration Fee will be refunded and this Enrollment Contract will be null and void. In
either event, you will only be charged for the number of actual school days up to the dismissal/withdrawal
date.)
Tuition: In consideration of the acceptance of this Enrollment Contract by MVM, the Responsible Party
agrees to pay the required fees as specified herein. Tuition & Extended Program Fees (“Tuition”) for the
2015-2016 school year shall be paid in ten (10) monthly payments in the amount indicated in this
Enrollment Contract. Monthly Tuition payments shall be due on the first of each month beginning August
1, 2015 and ending on May 1, 2016. Monthly Tuition payments shall be made in the form of check or
money order (MVM does not accept cash). Upon execution of this Enrollment Contract, a non-refundable
$500.00 Registration Fee shall be due and payable. Full payment of the Registration Fee and full
execution of this Enrollment Contract, subject to the Trial Period above, guarantees Student’s placement
in the program. There shall be a 5% sibling discount on Tuition only (not on Extended Program Fees) for
the second child (younger child). All prices are subject to sales tax.
Late Fees: The Responsible Party shall pay a late fee of 1.5 % (18% per annum), assessed on all Tuition
paid after the 5th day of the month. This fee shall be assessed cumulatively, every month in which there is
any unpaid balance due. After ten (10) days of non-payment, MVM reserves the right to dismiss the
Student from the school. Further action may include informing a credit bureau and taking legal action
against the Responsible Party to enforce debt collection. The Responsible Party shall be liable for any
and all court costs, fees, and attorney fees incurred to collect outstanding balances.
Enrollment Policies: The Responsible Party shall be obligated to pay Tuition for the entire school year.
In the event of Student’s withdrawal from the school, any Tuition balance shall not be refunded or
cancelled. The Responsible Party’s payment obligation shall be based on the program for which the
Student is enrolled, not on actual attendance. Additional charges may be billed to the Responsible Party
for field trips or special programs. (In the event that the Student is withdrawn from the school for any
reason, the Responsible Party will have 2 payment options: [1.] The Responsible Party may continue
paying the balance of the Tuition on a monthly basis as billed, or [2.] The Responsible Party may pay a
lump sum discounted amount on the last day of the Student’s attendance. This discounted amount will be
50% of the Tuition balance.)

Pick-up/Drop-off Times & Extended Programs: Tuition shall cover days and times that the Montessori
programs are in operation. There shall be a 15 minute drop-off grace period (for all programs) and a 15
minute pick-up grace period (for all programs except PM-Extended Program). If Student is dropped-off
early or picked-up late and is not enrolled in an Extended Program, there shall be a fee of $12/hour,
charged in 1 hour increments. (Note that there is no PM - Extended Program for Half Day Students.) Any
student picked up after 6:00 PM shall be charged $30/hour with a $30 minimum. Extended Programs
shall be set up front for the entire school year and available on a prepaid basis due to student-teacher ratio
requirements.
Acceptance of Rules & Hold Harmless Clause: The Responsible party shall accept the rules and
regulations of MVM as stated in the Parent Handbook and the rules concerning payment of Tuition as
referred to above. The Responsible Party shall release, and hold harmless, MVM, its agents and
employees from all claims, damages, and/or other liabilities for the injuries to the Student which are not
the result of gross negligence by MVM, its agents, and/or employees. The Responsible party shall
indemnify MVM for damages by the Student.
Media Consent and Release: The Responsible party hereby grants MVM the irrevocable right to use the
Student’s photograph, individually or in conjunction with other in-school related activities, for
reproduction in any print or electronic medium for purposes of advertising, trade, display exhibition,
competition, or editorial use including the MVM website, MVM Facebook, and any other social medium
used.
Programs: In addition to the regular 5 day programs, some of the programs (but not all) allow 3
and 4 day attendance, but the specific days are subject to availability and teacher approval. The
3 day attendance and the 4 day attendance must be consecutive days. Again, these programs
are subject to availability and teacher approval.
