Northern Diabetes Educator’s Nomination Form 2015-2016
It is time to call for nominations for officers for the Northern Diabetes Educator Group to fill position that will be vacated
in July of this year. Elections are in early April 2015.
This year NDEG is looking for a President Elect, as Kara Randall will be completing her two year term, a Secretary, as
Lisa Kuklis will be completing her two year term, an Industry Representative, as Beth Chesley will be completing her
two year term, and a Member-at-Large as Cathy Nelson will be completing her two year term.
As an incentive to collect nominations, NDEG will be giving away two $25 gift cards from a drawing of names of
those who were nominated and those individuals who nominated someone. AND those that accept positions will
receive (1) year of annual dues and (1) all-day conference ($110.00 value). The drawing will take place at the
annual NDEG Meeting on Friday April 17, 2015
How do you NOMINATE?
1. You can add name(s) onto this form and turn it into any current NDEG Officer
2. You can send an email to any NDEG officer (see email addresses below) with your suggested nominations.
3. You can mail this form with your nominations to NDEG, P O BOX 16834, Duluth MN 55816
NOTE: If someone is nominated, they will be contacted by a current officer to see if they are willing to be on the ballot
as an NDEG Board Member. Just because someone is nominated does not mean that they are automatically put on the
ballot. They will be contacted first.
2nd NOTE: NEW: Only Diabetes Educators are eligible to hold an officer position President/President Elect with the
NDEG Board. Business Industry ARE eligible for other officer positions. We want to thank the business industry
members for being so actively involved.
