All student athletes participating in the Foothill Cougar’s 2015 Volleyball Camp must have signed the permission slip (mail
this form in with your camp fee). In case of accident or injury you give permission for the supervising staff to authorize medical treatment by a licensed physician. You understand that Foothill High School and the Shasta Union High School District
are not responsible for any expense incurred as a result of obtaining necessary medical care.
Camp starts at 9 am. Please arrive early to sign in and receive a camp shirt. Camp will end at 2 ppm each day.
