Important Note: Carefully read and follow the steps listed below. Scan and Email all of your completed documents,
including this check-off sheet, to: [email protected] (PDF is preferred, but jpeg files will also be accepted).
We do not recommend sending in hard copies, however if you wish to submit paper documents, turn them into the main
Nursing Office (EC-190). We will only accept COPIES of your documents. Please submit your copies in an envelope with
your name, CWID #, and a phone number written on the outside.
KEEP ALL OF YOUR ORIGINALS until the end of the semester.
 LAMC Orientation Packet: Read over thoroughly (29 pgs)
 LAMC Student Test: Print out, complete, input your name and date (3 pgs)
 LAMC Cerner Application* : Print out, complete, sign and date (1 pg)
 LAMC e-Tenet Instructions: Read over and follow all of the steps carefully (1 pg)
 LAMC Parking Maps: Review and print a copy for future use (2 pgs)
 LAMC Parking Memo: Read over thoroughly (1 pg)
 LAMC Parking Signature Page: Print out, complete, sign, and date (1 pg)
* Please note that all students will be required to attend a mandatory Cerner Orientation onsite at the
Los Alamitos Total Care Pavilion. More information will be sent to you regarding the date and time.
