Pet Waiver for the 2015 NW Pet Fair
I wish to bring my pet into the NW Pet Fair (hereafter referred to as the “Event”). By attending the Event with
my pet, I release, hold harmless and indemnify the NW Pet Fair, West Columbia Gorge Humane Society, Living
Out Loud, Inc. dba Spot Magazine, its employees, owners, volunteers and representatives and any other
individuals or entities (whether or not professional) involved with the production of this Event, and any and all
attendees from any and all liability, damage, and/or claim of any nature whatsoever arising out of or in any
way related to my attendance/the attendance of my family, and/or of my pet* which I bring to the Event.
I recognize that I am solely responsible for any harm, injury (personal or otherwise), illness or damage caused
by my pet which I bring to the Event.
I recognize that by attending and/or participating is an activity which presents some risks of injury and/or
illness. I agree that the responsibility for the safety of myself, my family, my pet which I bring to the Event is
my responsibility; and not the responsibility of NW Pet Fair, West Columbia Gorge Humane Society, Living Out
Loud, Inc. dba Spot Magazine, its employees, owners, volunteers and representatives and any other individuals
or entities (whether or not professional) involved with the production of this Event, and any and all attendees.
By signing this Waiver, I confirm that (a) I have read, understand and freely agree with the foregoing terms,
(b) I am eighteen (18) years or older, and (c) I am legally competent to execute this Pet Waiver. I further agree
that I will comply with the Rules set forth on the following page, as well as the policies of the space in which
the Event is being held.

*For purposes of the Waiver and the Rules, the word “pet” may be read as singular or plural, or as male or female, as
required by the context.

NW PET FAIR RULES:
My pet is domesticated.
My pet is up-to-date on all age-appropriate vaccinations.
My pet is not of an aggressive nature.
My pet is properly fitted with a collar and leash.
I will pick up my pet’s solid waste as a courtesy to others and dispose of it in the on-site receptacles.
If female, my pet is not in heat.
To the best of knowledge, my pet is free from any contagious diseases or conditions that would make
it a danger to any participant, exhibitor, attendee, or other individual.
I shall immediately remove my pet from the Event if it acts aggressively towards other people and/or
their pet or animals; poses a safety risk to other people and/or their pet; or if my pet appears to be
unhealthy or becomes ill.
Without exception, I accept full financial responsibility for any damage(s) to persons, pets or
properties as a result of the behavior of my pet at this Event.
I also understand that the NW Pet Fair, West Columbia Gorge Humane Society, Living Out Loud, Inc.
dba Spot Magazine, its employees, owners, volunteers and representatives and any other individuals
or entities involved with the production of this Event, reserve the right to refuse admittance to any
unruly, aggressive, unsuitable or obviously ill pets. In addition, I also know that my pet may be asked to
leave the Event if it displays behaviors that are deemed to be disruptive or dangerous.
I agree to sign this Pet Waiver, releasing NW Pet Fair, West Columbia Gorge Humane Society, Living
Out Loud, Inc. d/b/a Spot Magazine, its employees, owners, volunteers and representatives and any
other individuals or entities involved with the production of this Event and any and all attendees of any
liability.
Failure to sign this Pet Waiver will preclude the attendance of my attendance or participation in the
Event.
By signing these Rules I acknowledge that I have read and understand the above Rules, and I agree to comply
with these Rules for the safety of others, myself and my pet.
