INFORMATION
Entry Fee:
$15.00 (youth 16 and under, $5.00)
You must be a NYSLA member to compete. If you are not a member, please enclose an additional $30.00 for
membership and complete the membership form, shirt order form and photo release form available at
www.nylumberjacks.org. A competition shirt will be provided as part of this fee and must be worn while
competing.

Deadline :
Entry forms and fees must be postmarked on or before April 16, 2015.
Entry forms must include accurate email address, partner’s names for doubles events, and equipment sharing
needs. Registration for doubles events is valid only if an entry form is received on time, with payment by the
named partner(s).
