All nominations will be verified with an e‐mail from our Conference Program Manager. If you have not received a verification e‐ mail by February 6, 2015, please contact Christy Jacobs at oaccac‐conference@mci‐group.com. Nominee photographs are to be submitted in order to be included in the Awards Video presentation that will be used at the banquet. Nominee photographs must meet the following requirements: JPEG format High resolution 300 dpi (ideally) For team nominations, the following are options for ways to submit photographs: Team photo Photos of the individuals (that will be used to create a collage) Logo/banner that represents the team Please forward to Christy Jacobs at oaccac‐conference@mci‐group.com, by April 3, 2015. Please reference the name of the nomination in your correspondence. The OACCAC will not take formal Nominee Photographs at the Nominee Reception. Informal photographs will be taken at the Reception, which is scheduled for May 28, 2015 at 6:00pm, Westin Harbour Castle, Frontenac Ballroom Foyer. The banquet is scheduled for the same day, at 7:00pm in the Frontenac Ballroom. 3 Nomination Guidelines AwardSubmissionGuidelines Please review these guidelines prior to filling out the appropriate online form below depending on the award category being entered. The Award categories, criteria and processes have been reviewed by the Awards Committee. Nomination submissions will only be accepted through the CCACs. Each CCAC is allowed only one nomination to each of the five award categories listed below: o Individual CCAC Staff o CCAC Staff Team o CCAC Leadership o CCAC Citizenship o Systems Partnership – with other CCAC(s) or outside organization(s) CCAC CEO Council will identify initiatives for which nominations will be prepared related to the: o Sector Innovation Award Previously unsuccessful submissions may be re‐submitted. General Eligibility Criteria for Nominees: o reside or work in Ontario o be in good standing with the CCAC/OACCAC o be associated with the CCAC/OACCAC organization (partnerships) Acquire permission from the nominee(s) prior to submitting the nomination. By submitting the nomination(s), the CCAC and nominee(s) agree to the publication of the submitted information by the OACCAC (i.e. for conference materials/publicity, posting on OACCAC’s website and Members Portal, use on screens at the conference). Support from the CEO and/or Board Chair is required to validate the achievements. It is recommended that nominations be reviewed by Communications staff and/or senior management before submitting online. Each CCAC is allowed up to 250 words under each criterion. It is recommended that the maximum amount of words be used/criterion. A separate online form must be submitted with each entry. All entries must be submitted electronically by end of day February 6, 2015 using the online form that will be posted on the OACCAC website. Ensure that the submission is entered under the correct category. The online process will allow you to save your work on the submission. However, once your nomination is "submitted", the nomination information that you provide will be considered "FINAL" and cannot be revised or altered. Nominations submitted after the submission deadline cannot be included in the award review and recognition process. Content from the Executive Summary section may be used for publication purposes. Three key points (in point form) can be listed as to why the nomination is outstanding. Be sure to inform all nominees of the award nomination. 4 Nomination Guidelines The CCAC will support its nominee(s) through the nomination, selection and recognition process. Support to include, but not limited to: o preparation and submission of nomination materials by the submission deadline o provision of photos of the nominee(s) (photo requirements outlined on page 3) via email to Christy Jacobs by April 3, 2015 o financial assistance when possible to send the nominee(s) to the banquet TIP: For ideas on who to nominate, you may consider individuals who have been recipients or nominees of other awards/distinctions within your CCAC or community. In addition to recognition at the OACCAC 2015 Awards for Excellence Banquet, up to 3 team members of the winning team (selected by the team members) will receive the following from the CHCA: All‐expense paid (registration, travel, accommodation, meals) trip to the 2015 CHCA Annual Home Care Summit The opportunity to present their initiative at the 2015 Summit The opportunity to participate in the development of a written CHCA high impact practice [CHCA High Impact Practices are evidence‐informed, innovative practices within the home and community care sector that enhance the quality and effectiveness of service and result in improved quality of life for clients and their families. By identifying and sharing High Impact Practices, the CHCA facilitates the dissemination of knowledge, expertise and experience.] 2) Please note that, in some cases, it may take approximately a minimum of 3 weeks to gather the information, prepare the background statement and to obtain consent from nominee(s). You may wish to meet with other individuals to help provide the evidence for creating the nomination. You need to make sure that you provide yourself with enough time to complete and submit your nomination. 3) Prior to accessing the online nomination form, consider the following: a. Have you provided a background in the executive summary regarding the nomination submission to support the overview of what the award nomination is for? Considerations may include how did this person/group/initiative commence? What is/are the unique characteristics that make this person/team stand apart from others? What are key areas that have not been highlighted by responding to the questions that you want to make sure that the Review Committee is aware of when they are evaluating this Award submission? b. What are the identified measurable outcomes that have been accomplished? What are the identified lessons learned and changes made as a result? 4) Have you clearly and effectively responded to each of the key questions noted in the checklist? 5) Have you spelled out any acronyms used since judges may not be familiar with them? At a minimum, the first time an acronym is used must be written out in full. 6) Do your responses highlight the key factors that are noted in the specific Award criteria for that category? 7) Have you ensured that you have contained the responses to each criterion in the appropriate section noting that responses will only be graded if placed in the appropriate section? This team will reflect membership from CCACs provincially, the OACCAC and other organizations and/or associations. The submission must be made only in the final year of implementation so that the significance of the outcomes achieved can be identified and related to the broad system level impact and transformation that has occurred.
