2015 Resident Teacher of the Year Award Nomination Instructions and Form The Resident Teacher of the Year Award recognizes outstanding teaching skills and commitment to education among family medicine residents currently enrolled in a Family Medicine residency program in Ontario. To be eligible a candidate must be enrolled in a Family Medicine Program during the 2014‐
2015 academic year and involved in teaching, education and activities that demonstrate an outstanding contribution to their peers, faculty of the Family Medicine Residency Program, and/or family physicians in practice. Nominations for this outstanding individual can be made by current Faculty members, medical students and residents, as well as family physicians in practice. The winner will be recognized at the OCFP’s Awards Ceremony in Toronto in November. The winner will receive a $1000 honorarium. Deadline for Nominations – April 10, 2015 INSTRUCTIONS Please include the following materials in the Resident Teacher of the Year Award nomination package:  The completed nomination form (see last page).
 A nomination letter that describes the nominee and provides examples of
demonstrated leadership, educational activities and skills that:
 Enhance the learning and development opportunities for medical students
and fellow residents;
 Demonstrate effectiveness as a role model for Resident colleagues;
 Support Faculty and family medicine as a discipline through educational
activities.
 The following skills and aptitudes: communicator; collaborator; educator;
mentor
 The nomination letter should be typed and not longer than two pages. Please use
bullets as appropriate.
 At least two letters of support from colleagues or preceptors (to a maximum of five
letters).
 A recent CV .
