SonSpark Labs Catholic 2015 Vacation Bible School Registration Form
June 22 through June 26, 2015
9:00 a.m.-12:00 p.m.
Vacation Bible School is open for registration on a first come first served basis. There is space
available for twenty children per grade level in the Kindergarten through Fourth Grades. Fifth
and Sixth Grade are a combined class and have space available for a total of twenty children at
ten children per grade level. If the class is full when we receive your registration your child will
be placed on a wait list, in case of a cancellation. The cost is $30.00 per child for the week. If
your child’s registration is accepted you will be notified by email. If there is no space available
your check will be returned to you on June 4, 2015. A letter of acceptance to the Vacation Bible
School Program will be emailed to the parents by June 4, 2015.
This summer, bring your kids to “SonSpark Labs Catholic Vacation Bible School where your
child will connect with God. Every activity is designed so that the children; hear, see, sing and
touch God’s Word. The children apply what they learn as they serve others and watch for God
in everyday life. At the amazing SonSpark Labs, children will explore God’s life-changing plan
as they find out the answers to life’s most important questions. They will discover that God loves
them and that through Jesus they can be members of God’s family and personally experience
God’s plan for each of us— GP4U=J!
Parent Volunteers are needed; in fact, we cannot hold Vacation Bible School without adequate
Adult Volunteers. For parents who volunteer all five days of Vacation Bible School there is no
cost for your children to attend. To apply as an adult volunteer you must call Dina Voutour 2852698 ext. 224. All Adults must be fingerprinted for the Diocese of St. Augustine, have a
thorough back ground check and attend a Protecting God’s Children three hour workshop prior
to volunteering. Additionally, all adult volunteers must complete and submit the 2015 Adult
Volunteer Application for Vacation Bible School work.
This program is open to children at least 5 years old that are entering Kindergarten the fall of
2015 as well as children through the Sixth Grade.

Due to limited space availability you must return the three completed forms (located below)
along with your check to the Parish Religious Education Office as quickly as possible.
Checks must be made out to Our Lady Star of the Sea Catholic Church.

Sea Church in Ponte Vedra Beach, Florida will not release any photograph without prior written
consent from you as parent or guardian.
