PARENTAL CONSENT FOR OTHERWORLD 2015 ________________________________

hereafter referred to as “the minor”, does hereby state that they wish to participate in the Otherworld 2015 arts event hosted by the Kindle Arts Society, hereafter “KindleArts” on June 26 to June 29, 2015. By signing this document, the minor’s parents or guardians acknowledge that they consent to the minor attending this event, that they will provide the required supervision, and that they are aware of the significant risks involved. These risks include, but are not limited to: toxic substances, pyrotechnics, fire, fire flow toys, and motor vehicles and art cars

The event site contains sufficient water to drown and objects high enough to lead to fatal falls. Structures and artworks may be in an unsafe condition. Parents or guardians should obtain their own health, disability and life insurance for the minor. Parents or guardians must supervise children aged 12 and under at all times by keeping the child in direct sight. Parents or guardians must monitor teenagers aged 13 to 18 years old at all times by knowing where their child is and what they are doing. All minors must be directly supervised by a parent or guardian while in the ocean. A parent or guardian must always be within arms length of a child 7 years or under while in the ocean. The minor may be exposed to nudity, sexuality, intoxication, and other adult situations. Areas of the event may be marked Adults Only and it is the responsibility of the parent or guardian to ensure minors comply with these notices and do not enter these areas. The minor agrees to: ● Adhere to the Otherworld Code of Conduct ● Refrain from becoming intoxicated ● Stay out of any areas marked Adults Only Without the consent of parents or guardians, the minor will be refused entry to the event and KindleArts will refund the minor’s ticket price. __________________________________
