RELEASE OF LIABILITY AND ASSUMPTION OF RISK
In agreeing to participate in any way in the Pack Swim Camp, LLC program known as the WOLFPACK SWIM CAMP its related
events and activities, you hereby acknowledge, appreciate, and agree to the following terms and conditions:
For yourself and on behalf of your heirs, assigns, personal representatives and next of kin, you HEREBY WAIVE AND RELEASE,
indemnify, hold harmless and forever discharge Pack Swim Camp, LLC its agents, employees, officers, directors, affiliates,
successors and assigns, coaches, teachers and trustees thereof from any and all claims, demands, debts, contracts, expenses,
causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity,
that you ever had or may have, arising from or in any way related to your participation in any of the events or activities
conducted by Pack Swim Camp, LLC or at North Carolina State University (hereinafter, “Premises”), or for the benefit of Pack
Swim Camp, LLC and, if applicable, North Carolina State University owners and lessors of Premises used for the activity
("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property
associated with your presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR
OTHERWISE, to the fullest extent permitted by law, provided that this release of liability does not apply to any acts of gross
negligence, or intentional, willful or wanton misconduct;
By participating in a swim camp or swim related activity, you realize and accept the fact that this requires physical exertion.
In addition, you realize and accept the fact that you are in good enough physical condition to participate in this event. You
agree to follow the rules and guidelines of the committee, volunteers and course security during this event. As a result, you
agree to hold harmless all sponsors, supporters, committee members, volunteers and benefiting organizations, including
North Carolina State University and Pack Swim Camp LLC. You, therefore, release these parties from all liabilities associated
with your participation in this event and associated activities; and
You willingly agree to comply with the stated and customary terms and conditions for participation. If, however, you observe
any unusual significant hazard during your presence or participation, you will remove yourself from participation and bring
such to the attention of Pack Swim Camp, LLC immediately.
YOU HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS,
UNDERSTAND THAT YOU HAVE GIVEN UP SUBSTANTIAL RIGHTS BY ACCEPTING THE TERMS OF SAID AGREEMENT AND DO SO
FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT TIME OF REGISTRATION)
You certify that as parent/guardian with legal responsibility for this participant, you do consent and agree to his/her release
as provided above of all the Releasees, and, for yourself, your child and your heirs, assigns, and next of kin, you release and
agree to indemnify and hold harmless the Releasees from any and all liabilities incident to your minor child's involvement or
participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the
fullest extent permitted by law.
