Site Induction Fact Sheet
To ensure your Booking Request can be confirmed, you and / or your company
are required to complete the following process and documentation.
MILLENNIUM MINE INDUCTION BOOKING PROCESS - ONLINE BOOKINGS (no manual bookings accepted)
1.
Lodge an induction application to [email protected]
2. After authorisation you will receive a link to direct you to the Pegasus Safety portal.
3. Register your company and the individual who is to undertake the induction and upload FOUR documents and a photo. Follow
the instructions as provided.
4. After successful verification of these documents, the inductee will receive a link and log on ID and password via email.
5. The inductee then needs to complete the elements in the Online Induction Millennium – Contractor or Employee as
appropriate.
6. Once they have completed 100% of the online component (it will say 100% complete) they can then apply for a date to
attend the site component of the induction. They do not need to bring the documentation uploaded with them.

On completion of both components they will receive an access card at site allowing them access to the mine.
8. The card must be used to gain entry to the mine and on departure. If they do not log out they will be called to ensure they are
no longer on site no matter the time of the day or night.
9. Those contractors that are currently inducted but do not have a card need to apply for one through the HST department. They
will need to supply a passport photo of head and shoulders against a neutral background without hat or glasses. The contact is
the same as the inductions email above.
DOCUMENT REQUIREMENTS
DESCRIPTOR
Drivers Licence
(provide on a separate piece of paper)
Government body Drivers Licence, all states accepted.
Colour copy, both front and back to be uploaded.
Standard 11 Mining Induction
Card and certificate to be supplied.
Coal Board Medical Certificate
Must be less than 5 years old.
Drug and Alcohol Test
No more than 7 days old.

Payment Options
Approved account holders may use a P/O, otherwise credit card payments online
are the only accepted method.
Documentation Submission
Documents to be scanned one per page, clear,legible and in colour.
