PARENTAL/GUARDIAN CONSENT FOR ACTIVITY PARTICIPATION
Please complete the details below if you would like your child to participate in the
following activity. This form must be returned to Julie Bayford at Phoenix Group for
Deaf Children prior to the activity. Our new address is the Hyde Out Centre, 2-6 the

young people taking part live.
