Camp Objective
This camp is designed to emphasize improvement of fundamentals and techniques related to each
camper’s position and skill level. Campers will be coached in offensive, defensive, and speed/agility
fundamentals. Experience playing organized football is not a requirement for attendance of this camp.
All Campers are grouped by age level and ability. We strive to give our younger campers a solid
well-rounded curriculum that allows opportunities for participation with a variety of positions and
skills. We try to take our older and more experienced athletes to the “next level.” Emphasis with these
campers is placed on teaching those extra little things that will give them the edge. Particular attention
is given toward increasing the self-esteem and psychological development of each camper. Our goal is
for each camper to have a safe and positive experience. We stress individual attention, improving skill
level, and having an enjoyable time!
The camp includes 5 practice sessions of instruction on football skills, cardiovascular activity, and
team building concepts. A flag football tournament will be held on Friday. The camp will be
instructed by the Poway Football coaches and players.
What to Bring
Campers are encouraged to bring water, sunscreen, a mouth piece, and cleats or athletic shoes (cleats are not mandatory).
Admission of Camp Spectators
Those who would like to attend the camp and view camp activities as a spectator must be seated in the bleachers at all
times. Under no circumstances are visitors allowed on the track, playing surface, or the weight room.
Tuition $100-The non-refundable check or cash is due June 1, 2015 to guarantee a camp t-shirt.
Walk up registration is allowed on June 16th at 8:00 am. Please have the medical information form and
waiver ready. Sibling discount for each additional camper is $50.

I do hereby, and on behalf of Attendee, give my consent to Damian Gonzalez and the staff of the Titan Football Club
Camp to act for me according to their best judgment in any emergency requiring medical attention if such treatment
should be necessary or desirable during the course of Attendee’s participation.
ASSUMPTION OF RISK- I do hereby, and on behalf of Attendee, accept, understand and assume that that participation
in the Titan Football Camp activities carries with it certain inherent risks that cannot be eliminated regardless of the care
taken to avoid injuries. I do hereby, and on behalf of Attendee, accept, understand and assume that the Titan Football
Camp activities carry with them inherent risks due to the physical nature of the activities which include but are not limited
to, physical contact with other participants, footballs being thrown, physical drills, and activities related to playing a
physical sport like football. I do hereby, and on behalf of Attendee, accept, understand and assume that these risks include
minor injuries such as scratches, bruises, sprains, more serious injuries, including possible permanent physical and/or
mental damage, heart attacks, concussions, and even paralysis, or death. I do hereby, and on behalf of Attendee, agree that
Attendee has agreed to follow all instructions of Coach and Camp Staff, and to wear all necessary, recommended, and
appropriate protective gear and equipment during the course of the Titan Football Camp. I do hereby, and on behalf of
Attendee, agree to indemnify and hold Coach, Camp Staff, Camp Sponsors, and PUSD harmless from any and all claims,
actions, suits, procedures, costs, expenses, damages, liabilities, and any attorney’s fees brought as a result of Attendee’s
involvement in the Titan Football Camp, and I agree to reimburse the Coach, Camp Staff, Camp Sponsors, and PUSD for
any such expenses incurred by them.
My signature also certifies that my child has accident medical insurance and has obtained proper medical care for any
current medical condition for the duration of the Titan Football Club Camp.
I have read this document in its entirety, fully understand its terms, understand that I have waived substantial rights by
signing it, and sign it freely, voluntarily, and without inducement. By my signature below I am verifying and warranting
that the Attendee does have such coverage. I certify that I am the parent or legal guardian of the child Attendee listed
above. I acknowledge I have carefully read the full contents of this “Waiver of Liability, Release, Assumption of Risk,
Indemnity and Hold Harmless Agreement,” that I fully understand its contents, and have signed below on my behalf and
also on behalf of Attendee, of my own free will.
