Rehearsals
YMCA Dance Studio and on stage at the theater.
Beginning in January, choreography rehearsals will be conducted during the last part of
the students’ classes so as not to interrupt their regular training. Some groups may be
scheduled for Saturday rehearsals as we get closer to the date. We will notify you in
advance so families can plan accordingly.

Pictures will be available for purchase from the photo company with a pre-paid
order envelope available before the photo shoot.
Class Attendance / Monthly Tuition
Dancers will need to attend their monthly class/classes and keep current on their
monthly class fees.

Costumes
Costume delivery and parents meeting week: May 4-8
One parent is required to attend their child’s class/classes during the week to pick-up
costumes, receive information about hair, make-up and accessories.

I have read and understood the guidelines as presented and agree to the following:
To have my child participate in the rehearsals and performances.
To have my child ready and on time for all scheduled Rehearsals and Performance.
To have my child in appropriate dress code.
I understand that the YMCA cannot supply transportation to performances.
I give permission for photos/videos to be taken of my child for marketing purposes.
I understand that recital fees are non-refundable / non-transferrable.
In case my child is absent from a rehearsal or performance because of a contagious
illness, injury or family emergency, I will immediately contact Dance Coordinator Alison
Black.
I understand that May tuition will not be altered due to parent meetings, rehearsals or
performances. I also understand that recital fee and any other balance on account must
be paid before my child/children may participate in the rehearsals and performances.

RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT
In consideration for being permitted to utilize the facilities, services, and programs of the Prescott Young Mens
Christian Association of Yavapai County ( the”YMCA”) for any purpose, including but not limited to observation
or use of facilities or equipment, or participation in any program affiliated with the YMCA, without respect to
location, the undersigned, for himself or herself and any personal representatives, heirs, and next of kin,
hereby acknowledges, agrees and represents that he or she has, or immediately upon entering or participating
will inspect and carefully consider such premises and facilities or the affiliated program. It is further warranted
that such entry into the YMCA for observation or use of any facilities or equipment or participation in such
affiliated program constitutes an acknowledgement that such premises and all facilities and equipment thereon
and such affiliated programs have been inspected and carefully considered and that the undersigned finds and
accepts same as being safe and reasonably suited for the purpose of such observation, use, or participation.
IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER THE YMCA FOR ANY PURPOSE,
INCLUDING BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR
PARTICIPATION IN ANY PROGRAM AFFILIATED WITH THE YMCA, WITHOUT RESPECT TO LOCATION,
THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:
1. THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE
the YMCA, its directors, officers, employees, and agents (hereinafter referred to as "releasees") from all liability
to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damage, and
any claim or demands therefor on account of injury to the person or property or resulting in death of the
undersigned, whether caused by the negligence of the releasees or otherwise while the undersigned is in,
upon, or about the premises or any facilities or equipment therein, or participating in any program affiliated with
the YMCA, without respect to location.
2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the
releasees and each of them from any loss, liability, damage, or cost they may incur due to the presence of the
undersigned in, upon, or about the YMCA premises or in any way observing or using any facilities or
equipment of the YMCA or participating in any program affiliated with the YMCA whether caused by the
negligence of the releasees or otherwise.
3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY,
DEATH, OR PROPERTY DAMAGE due to negligence of releasees or otherwise while in, about, or upon the
premises of the YMCA and/or while using the premises or any facilities or equipment thereon or participating in
any program affiliated with the YMCA.
THE UNDERSIGNED further expressly agrees that the forgoing RELEASE, WAIVER AND INDEMNITY
AGREEMENT is intended to be as broad and inclusive as is permitted by the law of the State of Arizona and
that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full
legal force and effect.
THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY
AND INDEMNITYAGREEMENT, and further agrees that no oral representations, statements, or inducement
apart from the foregoing written agreement have been made.

What better way to make your dancer feel special on the big day than to send an
encouraging note of love! Whether you’re out of town or in the front row seats, wishing
him/her luck in a memory filled program will give him/her the opportunity to read the
note over and over as he/she reminisces through the years. From businesses to long
distance relatives, everyone can support your dancer on this exciting day!
Messages (20 words max) may be on a ¼ page minimum with a head shot picture of
your dancer or 1/2 page with a full length picture in costume.

2) Katy - ‘We are proud of you and enjoy watching you dance. Keep up the good work!

Special Note:
If you are willing to help us in acquiring one or more ads for the Recital 2015 Program
souvenir, we request that you submit a list of the names of persons and businesses you
plan to contact to [email protected] before making a contact. We check
these names against the list of individuals and businesses that are current donors to the
Y, so as to avoid asking those individuals and businesses for another donation. You will
receive a prompt reply from us.

make Recital 2015 an extraordinary event.
