PRIMARY Field Trip Permission Slip
Summer School 2015
Our summer school is filled with exciting and fun adventures. Below you will find a field trip schedule outlining all field
trips and special events. This form must be submitted a week in advance of the field trip date. Please check the
field trip that corresponds with the weeks you have registered your child for summer school. Your child may not
participate in any of the field trips if this form is not submitted to the office in a timely manner.
If you do not want your child to attend a certain field trip, or you need to make changes/cancellations, please notify the
office at least 5 days in advance. If we do not receive your cancelation a week prior to the field trip, we are unable
refund payments. No exceptions will be made. Also, there will be no same day drop-ins for child care accepted on field
trip days.
All students ride a school bus to be transported back and forth to the site of the fieldtrip, unless otherwise specified.
For the safety of the children, all students are required to wear their school t-shirt on all field trip days. When lunch is
not included, all students must bring their own sack lunch.
