Medical Providers | EHR Connectivity
947 licensed providers, physicians/NPs/CNMs/PAs others include dentists, advanced practice nurses and other
licensed disciplines. 26 EHR products interfaced / in development with QHN. 211 clinics/practices have an interface
with QHN, 49 have bidirectional exchange. More than 90% of the medical providers in the western Colorado
medical trade area are connected to QHN.
• 5 EHR practice interfaces are in progress (3 outbound and 2 inbound)
Practice Transformation / Transitions
of Care: Registry / Risk Stratification Tool
• Population Health Management: Utilizing QHN’s HIE, 357 Providers (39 practices and clinics), benefiting more
than 340,500 patients use a registry / risk stratification tool for clinical decision support.
• 951 Providers and 130 Organizations utilize QHN Direct for secure messaging and Transitions of Care (ToC)
• 36 Practices/Organizations receive ED registrations and Admit / Discharge Alerts (ADTs) through QHN
• > 1 Million data repository, Patient Summary server requests (Year of 2014)
• Alerts/notifications functionality is currently in development via EHR
interface with numerous practices
