Please read the camp information, fees and dates carefully.
Please fill out the registration form completely, as no incomplete forms can be processed.
Registration: Onlye students in the appropriate grades are allowed at each camp. Space is limited for each camp and
registration is on a first-come, first-serve basis.
Registration Deadline: The Wednesday before each camp or when the max number of students has been reached.
Cancellations: Cancellations may be made 24 hours or before in writing to [email protected] for a full refund minus
a $35 processing fee. Refunds will not be given to any cancellations the day of or for no-shows; however, registrations are
transferable to a different camp or camper for this year’s camp season.
CHECK-IN & DEPARTURE: Check-in begins at 5.00pm on the Wednesday of each camp at the Realife Student Center at
either James River Church/South or West Campus, with a departure immediately following the evening service.
RETURN: Students will return approximately at 4:00pm on Sunday afternoon at the Realife Student Center, James River
Church/South Campus ONLY.
MEDICATION & FIRST AID [MEDICAL INFORMATION REQUIRED FOR 2015]:
A Health History form (included with Registration Form) will require a doctor’s signature in order for your child to
receive any over-the-counter medications as needed per stock while at camp from the first aid. All medications (prescription and/or over-the-counter), vitamins, herbs, and enzymes MUST have a doctor’s order and be brought in the
original pharmacy labeled container or the original manufacturer’s container to the first aid station at check-in.
Prescription medications MUST have the camper’s name on the bottle. Please make note on the medical form and alert
medical personnel at the first aid station during check-in of all allergies or medical conditions.
In order to provide coverage for our licensed medical professionals that volunteer at the James River Campground,
it was determined and verified by the American Camp Association, the Association of Camp Nurses and the Missouri
Scope of Practice of RN’s that our healthcare professionals need to have a doctor’s signature in order to provide any over
the counter medications including but not limited to: Ibuprofen, Tylenol or Zyrtec, Benadryl, and that any prescription
medication must be in the original container with prescription and dosage from the pharmacy which dispensed it.
If your child(ren) previously submitted this form for Spring Retreat in 2015, a second form is not required within this calendar year. *Additional details can be found online at realifeonline.com or by contacting the Realife Offices at 417.581.5433
Late Arrival/Early Departure: Early departure will be for emergency situations only. Only an authorized person listed on
the registration form may remove a student early.
Retreat Center Location: An hour and a half east of the James River Church/South Campus (30 minutes Northeast of
Mountain Grove, MO).
Phone Policy and Retreat Mail: Students will not be allowed to make or accept phone calls unless it is an emergency. We
are not responsible to forward late arriving mail. When writing to your child at camp, please send your letters to: (Student’s

Visitor Policy: All Camp activities are open only to registered students and staff. Visitors coming must receive
permission from the Realife Office prior to visiting and must check-in upon arrival. Must abide by visitors policy.
Visitors will be dismissed after the evening service lets out.
Retreat Center Property Damage: Charges for items broken/damaged or stolen during camp will be billed to the parties/
individuals involved.
Transportation: Students will be transported to and from the Retreat Center in school buses.
Meals: Campers will be provided three meals a day (two on Sunday) during their stay at the Retreat Center. However, Dinner
WILL NOT be provided the Wednesday night that camp starts. Please make arrangements to provide dinner for your camper
before check-in.
Scholarships: If applying for a scholarship, parents will need to complete the Scholarship Application along with the Registration Form. Scholarship Applications can be found at any JRC Guest Services or printed out at
www.realifeonline.com.
What TO Bring:
• Sleeping bag or bedding for a twin size bed
• Pillow
• Towels
• Personal hygiene items
• Clothing (see Camp Dress Code below)
• Bible/notebook/pen
• Camera (optional)
What NOT to Bring:
• Cell phone or other personal electronics
(i.e. MP3 players, iPods, Videogames, etc.)
• Inappropriate clothing
(see Camp Dress Code below)
• Firearms, knives, fireworks or explosives
• Water guns or water balloons
CAMP DRESS CODE: “MODESTY”
• All dress will be modest and appropriate
during camp
• Shirts: No spaghetti straps or shirts that show
the stomach
• Shorts/Pants: at least mid-thigh length, no
sophies; no yoga or stretch pants/leggings without
proper cover-up
• Jacket or sweatshirt (for cooler weather)
• Shoes: shoes must be worn at all times; it is suggested to
bring a couple of pairs of shoes - old tennis shoes that can
get muddy and ruined, plus an additional pair of shoes for
normal activities. Sandals and/or flip flops are acceptable for
non activity times
• No clothing with questionable content
• Camp director and leadership team will be the final
authority on the dress code
• Clothing for services (shorts are allowed)
RULES & REGULATIONS:
These rules are given as guidelines for every student to follow. This code of conduct has been established for your protection and
the benefit of every person present. They must be obeyed at all times.
• Each student must agree to abide by the dress code at all times.
• The daily schedule must be observed by all. Attendance at all camp activities is required of all
students unless granted permission by the camp director.
• No one is allowed to leave the retreat center without specific permission from the camp director.
• The possession or use of tobacco, drugs, or alcohol is forbidden and will result in dismissal.
• We reserve the right to inspect contents of all personal belongings. The holding and/or disposal
of improper contents is the right of the camp staff.
• There is to be no food or drink in the chapel.
• No PPDA (public or private display of affection).
• All staff members are authorized to maintain order anywhere on the grounds.
• Each student will perform his or her duties as part of the privilege of being at Camp. Rooms and
adjacent grounds must be kept clean daily.
• Each student is expected to observe habits of personal cleanliness, courtesy, and Christian conduct.
• Profanity is not allowed.
The lack of cooperation, unnecessary roughness, lack of respect for property, or an unwholesome attitude on the part of any
student will result in expulsion from camp. Parents/Guardians are asked to help in explaining these rules to their children
and in encouraging their compliance while at camp. Arrangements and expense of transporting expelled students home
from camp is the responsibility of the parents/guardians.
