Waiver and Release
The Gridiron High School Football Combine is a voluntary, athletic initiative sponsored by St.
Luke’s University Health Network and the Lehigh Valley Steel Hawks designed to test the
football and athletic skills of football players ages 13 – 18.
I wish to participate in the Gridiron High School Football Combine and related events. I
understand there are risks inherent with participating in athletic activities. I also understand that
during the Gridiron High School Football Combine, there will be a large number of football
players some of whom are inexperienced, creating further hazards. I voluntarily agree to
participate in the Gridiron High School Football Combine with knowledge of the hazards and
personal risks involved and accept all risks of injury, inconvenience, harm, loss, damage or
death.
In agreeing to this release, I acknowledge that I understand its intent, and I for myself, my heirs,
executors, administrators, assigns and personal representatives, agree not to sue and fully
release and forever discharge ST. LUKE’S UNIVERSITY HEALTH NETWORK, together with its
subsidiaries and affiliates and each of their officers, trustees, employees, agents, and
contractors (collectively, the “Network”) and the Lehigh Valley Steel Hawks and all other event
sponsors (together with the Network and the Lehigh Valley Steel Hawks, the “Sponsors”)
singularly and collectively, from and against any liability, causes of action, damages, claims,
costs, or expenses (including attorney’s fees), which may result from or be connected in any way
to my participation in the Gridiron High School Football Combine.
I represent that I am physically capable of participating in the Gridiron High School Football
Combine and the equipment I will bring with me, if any, will be in proper working condition. I
acknowledge that I, and I alone, am solely responsible for my personal health and safety, and
the personal property I bring with me. I agree to utilize any and all safety equipment I bring with
me or made available to me during the Gridiron High School Football Combine. I will observe all
posted rules and guidelines. I agree to abide by policies, rules and regulations established by
the Gridiron High School Football Combine event organizers and personnel, and conduct myself
in a safe and prudent manner while participating in the Gridiron High School Football Combine
events. I consent to and permit emergency medical treatment (to the extent it is available) in the
event of illness or injury, including transportation to a medical facility, and will be responsible for
any and all related costs.
I hereby consent and authorize the Sponsors to take photographs of me and interview me, and
to make video and/or audio records of me (the “Material”). I authorize the Sponsors to use the
Material for promotional purposes or any other purpose. I further release the Sponsors from any
and all liability arising from or connected with the taking, use, disclosure and distribution by the
Sponsor or any other party of the Material.
PARTICIPANTS UNDER 18 YEARS OF AGE MUST HAVE A PARENT OR LEGAL
GUARDIAN SIGNATURE IN ORDER TO PARTICIPATE.

child to participate in the Gridiron High School Football Combine, and that I agree on behalf of
myself and my minor child to the terms of this Waiver and Release.
