Understanding Safe Haven Hospice
Where is hospice care provided?
Safe Haven Hospice will provide care within
the patient’s home, whether in a private
residence, an assisted living or nursing facility,
or in the hospital.
Who pays for hospice services?
Medicare, Medicaid, and most private insurances
pay for hospice services.
What will Safe Haven Hospice provide?
Safe Haven Hospice will provide items related
to the terminal diagnosis including:
• Medications - prescription and
over-the-counter medications
• Durable medical equipment - such as
wheelchairs, walkers and hospital beds
Services provided include:
• Nursing services - routinely available and/or
on call 24/7, 365 days a year
• Medical social services
• Physician services including attending physician
• Counseling services including spiritual,
bereavement, and dietary
• Hospice aide and homemaker services
• Volunteer services
• Physical, occupational, and/or speech therapy
may be provided in certain circumstances
• Pet Therapy
• Healing Touch
Who may receive hospice services?
Hospice is for people who have a medical
prognosis with a life expectancy of 6 months or
less, if the illness runs its normal course.
Some signs and symptoms of terminal illness are:
• Difficulty swallowing and/or 10% weight loss
• Loss of mobility and increased dependence
• Frequent ER visits/hospitalizations
Safe Haven Hospice does not deny services to anyone
on the basis of race, color, national origin, religion,
age, ability to pay, and/or physical or mental handicap.
What levels of care are available?
Safe Haven Hospice is able to provide a variety
of levels of care to it’s patients. These include:

How can I begin to receive hospice
services?
After you contact Safe Haven Hospice the
following steps occur:
• We will perform a free medical evaluation to
see if the individual is appropriate for
hospice care
• We will contact your physician and our
Hospice physician
• If appropriate, the patient or family will sign
a statement electing hospice care
• For the duration of an election of hospice
care, an individual waives all rights to
Medicare payments that are related to the
terminal condition
