CWE SUMMER DAY CAMP – ROGERS at First Baptist Church on Olive Street 2015 INFORMATION SHEET PLEASE KEEP FOR YOUR RECORDS Family Orientation will be on the Sunday prior to the Session and is required. Session DC1: June 1 - 12 Session DC2: June 15 - 26 Session DC3: June 29 – July 12 Session DC4: July 13 - 24 Session DC5: July 27 – Aug 7 Each session is for a 2 week period, Mondays – Fridays only CWE Summer Day Camp - Rogers is a day camp program that takes place in Rogers, Arkansas, at the First Baptist Church on Olive Street. Daily program begins at 7:30 a.m. and concludes at 3:30 p.m. A fully completed application for each child is required to be considered for enrollment. If accepted, an acceptance letter and packet of necessary forms will be mailed to the applicant. HEALTH FORM Each camper must have a completed health form. A physical examination must be performed (on the Camp War Eagle health form) by a licensed M.D., N.P., P.A., or C.N.M. within 24 months of arrival at CWE Summer Day Camp. The Health Form should be returned to Camp War Eagle as soon as possible, but no later than the deadline on your acceptance letter. Campers will meet a nurse upon arrival at camp and leave any medication with the nurse. ALL HEALTH FORMS MUST BE SIGNED BY A LEGAL GUARDIAN (NO EXCEPTIONS). FEE PAYMENT All fees are due and payable to Camp War Eagle on or before April 1, 2015. Should you, for any reason, be unable to complete fee payment by April 1, 2015, alternate arrangements must be made through the Camp War Eagle @ The Jones Center office in Springdale. After April 1, 2015, all enrollments are subject to cancellation unless fees have been paid in full or alternate arrangements have been made with Camp War Eagle. FAMILY ORIENTATION Parents and day campers are required to attend Family Orientation, which takes place on the Sunday prior to their day camp session, from 2:00pm to 3:00pm, at the First Baptist Church at 626 West Olive Street (8th & Olive) in Rogers. OPENING DAY/ DAILY DROP OFF and PICK UP On Opening Day (Monday), CWE Summer Day Camp staff will be in the hallway on the east side of the building, facing the playground. Check-in will be open from 7:15 a.m. until 8:00 a.m. The First Baptist Church is located at 626 West Olive Street (8th & Olive), in Rogers. Campers will arrive by personal vehicle. Drop off will begin at 7:30 a.m. Once you’re officially checked in for the session parents will be able to drop off their campers at the same door on the east side of the building, facing the playground. Parents will not need to park and check their campers in daily. Pick up will begin at 3:30 p.m. It is necessary for you to be prompt in picking up your child(ren). You MUST pick up your child(ren) between 3:30 p.m. and 4:00p.m. During Orientation, you will be given a sign to display on the driver’s side dashboard of your vehicle, with your camper’s name on it. This identification is necessary for pick up of your student. If you do not have this document with you at pick up, you will be asked to park and enter the building, with some form of identification, to verify your right to pick up your camper. Your child(ren)’s safety is paramount to us, and we appreciate your understanding. ATTENDANCE POLICY If your camper is unable to attend day camp, for any reason, you must notify the CWE Day Camp Staff. After two (2) unexcused absences, your camper will be removed from the program and his/her spot will be filled by the next child on the waiting list. This policy is in place for the purposes of safety and camp participation opportunity for the many children on waiting lists. Refunds will not be issued for the program balance if you are in violation of the attendance policy. If your day camper will not attend camp on a particular day for an appointment, please send a note ahead of time, and remind the staff the day before, if possible. Campers are not allowed to come and go during the day. Campers are also not permitted to arrive to a session late or leave while camp is in session. Exceptions are allowed at the sole discretion of the Camp Director and must be cleared in advance. No tuition or fees will be refunded due to late arrivals or early departures. TELEPHONE USE In order that the camp routine is not disturbed, campers are not allowed to receive or make telephone calls except in the event of a medical emergency. Legal guardians should feel free to contact the Camp Director if they need to discuss something regarding their child. Cell phones may not be brought to CWE Summer Day Camp. If brought, will they be confiscated and returned to a guardian at the end of the camp day. ACTIVITIES Campers will be participating in the best that Camp War Eagle has to offer! Their days will include: Football Sculpting Kickball Concoctions Playground Games Leatherworking Softball Shrinky Dinks Cooking Basketball Volleyball Baseball Soccer Swimming These items are needed DAILY; please send packed in a bag or backpack: Swimsuit (one-piece swimsuits required for girls) Bible Towel Extra change of clothes Flip-Flops or water shoes Tennis shoes (should be worn to camp every day) Water bottle WHAT SHOULD YOU BRING TO CWE Summer Day Camp? IMPORTANT: EVERY ITEM THAT YOUR CAMPER BRINGS TO CAMP MUST BE MARKED WITH THE CAMPER’S FIRST AND LAST NAME. The following items should NOT be brought to Day Camp: Any profane shirt, including shirts that advertise or promote violence or alcohol/drug/tobacco products Electronic games, trading cards, marbles, toys Cell phones, MP3 players, iPods, etc. Any items of value, including money & jewelry ***CAMP WAR EAGLE IS NOT RESPONSIBLE FOR LOST OR STOLEN PROPERTY.*** We thank you for your understanding of our registration policies and procedures.
