DID YOU KNOW? The Cancer Assistance Program (CAP) was launched in 1994, by Don Muir and a group of dedicated volunteers, in order to meet an identified need of individuals with cancer, living on Hamilton Mountain. CAP now provides services for all of the greater region of Hamilton and surrounding communities. CAP receives no government funding. It is entirely dependent upon support from the community. CAP relies primarily upon volunteers for everything from planning to service delivery. Over 100 volunteers contribute their time, every year, to plan for and run fundraising events and deliver CAP’s much needed services. On an annual basis, CAP ·· provides services for over 2000 individuals ·· provides free parking for more than 1000 appointments taking place at the Juravinski Hospital or Juravinski Cancer Centre ·· provides over 3000 rides to and from cancer-related medical appointments ·· issues well over 5000 pieces of equipment, other necessities (e.g., wigs, breast prostheses and bras, bed pads, incontinence products, etc.) or oral nutritional supplements All of CAP’s services are offered free of charge for as long as they are needed. The Cancer Assistance Program is a community-based, not-for-profit organization, led by a team of volunteers and staff, offering free services to ease the physical, emotional and economic impact of those in our community affected by cancer. Its vision is of a community where people affected by cancer receive assistance and support in their time of need. TRANSPORTATION TO AND FROM CANCER-RELATED MEDICAL APPOINTMENTS Transportation is available, free of charge, to and from cancer-related medical appointments. All rides coordinated through CAP’s Drive & Ride program are provided by compassionate and trained volunteers. To register for transportation support, simply call the office, once you have a date(s) for which you require a ride(s). Transportation is available for residents of the greater region of Hamilton only Drive & Ride program proudly supported by PARKING CAP has limited parking available for individuals with appointments at the Juravinski Cancer Centre or Juravinski Hospital. To register for parking, you must call or attend the office upon having dates for which you will require a parking spot. Prior to using the parking, you must attend the office to pick up and sign for your parking pass. PERSONAL CARE AND COMFORT PRODUCTS Individuals can secure wigs, breast prostheses and mastectomy bras, at no cost. Monthly issuances of adult incontinence products and oral nutritional supplements are also available, at no cost. To register and secure the required items, the person in need or a support person must attend the office. HOME HEALTH EQUIPMENT LOANS Home health equipment loans are available, at no charge, for as long as individuals are in need of the same. Individuals can borrow as many items as required and exchange them as necessary, at any point that personal needs change. Equipment available for loan includes such things as: ·· Raised toilet seats ·· Bathtub transfer benches ·· Commodes ·· Bath chairs ·· Wheelchairs ·· Transfer chairs ·· Versa frames ·· Rollators ·· Walkers ·· Bed rails ·· Canes All equipment is sanitized and tested for safety prior to loan. To register for an equipment loan, the individual in need or a support person must attend the office.
