CASMI GOLF OUTING, June 4, 2015
£ I would like to sponsor ___ Golf Tee(s) at $150 per hole
Only twelve (12) of sponsorships are available – 8 have been sold. Tee sponsors will receive a custom sign with the company’s logo and
be listed on the CASMI web site.
£ I would like to sponsor ___ Hole(s)-in-One at $250 per hole
Only four (4) of sponsorships available – 2 have been sold. Each Hole-in-One sponsor receives a custom sign featuring the logo and will
be recognized on the CASMI web site.
X I would like to sponsor custom golf balls at $800 – SOLD
Only one (1) golf ball sponsorship available. All golfers will receive a sleeve of custom golf balls printed with logo. The sponsorship must
be paid by April 15th in order to produce and deliver. Sponsorship will be recognized on the CASMI web site.
X I would like to sponsor the Golf Goodie Bags at $1,000 – SOLD
Only one (1) goodie bag sponsorship will be available. As Goodie Bag sponsor, logo will be printed on bags stuffed with golfing “goodies”.
Sponsor will be recognized at Dinner and will have logo displayed on the CASMI web site. This must be paid by April 15th in order to allow
adequate time for production and delivery of the bags.
£ I would like to sponsor the Golf Beverage Carts on the course at $1,500
Only one (1) golf cart beverage sponsor available. As Beverage Cart sponsor, I will be recognized at registration, on the course with a
custom sign and will have my logo displayed on the CASMI web site.
X I would like to sponsor the Outing Golf Carts at $1,500 – SOLD
Only one (1) golf cart sponsorship available. As Golf Cart sponsor, I will be recognized all day at registration, at the first & tenth tees,
during dinner, and will have my logo displayed on the CASMI web site.
£ I would like to sponsor the Open Bar at the Golf Dinner for $2,000 – SOLD
Only one (1) open bar sponsorships is available. As Open Bar sponsor, I will have my logo displayed at the bar and on the CASMI web
site. I will also be recognized all day at registration and during the dinner with custom signs.
£ I would like to sponsor the entire Golf Dinner at $3,000

Dinner sponsor, will be recognized all day at registration, as well as during the dinner with custom signs and will have your logo displayed
on the CASMI web site.
