2014-2015 Award Nomination Form
Each year, the Office of School Counseling and Postsecondary Advising (OSCPA) recognizes outstanding school
counselors and college and career coaches that have clearly demonstrated excellence as well as administrators,
district/network level staff who have demonstrated strong support and partnership of school counseling and
postsecondary advising programs through a variety of awards issued at the Annual Celebration Luncheon for School
Counselors and Coaches, which will occur on June 5, 2015.
The People’s Choice Award recognizes a School Counselor or GEAR UP Coach who made a difference in the life of
a student, family, or a school community during the 2014-2015 school year.

ELIGIBILITY CRITERIA
The nominee must have served as a school counselor or college and career coach in Chicago Public
Schools during the 2014-2015 school year.

Individuals may submit multiple nominations for
a particular award, though a separate online
nomination form must be completed for each
award nominee.
Self-nominations will not be considered.
Incomplete nominations will not be considered.
Please ensure that you have submitted all
required information in sufficient detail.
Nominations received after 5:00PM on Friday,
May 15, 2015 will not be considered. This is a
firm deadline.
