Enrollment Form
Enrollment is on a first come, first served basis, and space is limited. To enroll, please
complete this form and send it with payment to Seattle Academy. Once your payment and
form are received, a confirmation letter will be sent to you. Fees are refundable up to two
weeks prior to the beginning of the program. Summer courses are non-credit courses.
Sessions not meeting minimum enrollment by May 1 will be cancelled and fees refunded.
Registrations will continue to be accepted after May 1 until classes are full.
